Showing all 23Employee utilizing hammer to straighten bracket, assisting another employee. When the other employee hit the opposite side of bracket the energy caused the bracket to bounce up and hit employee across the lip. Causing laceration and broken tooth.
Employees were removing chute work below the #7 screen. While supporting the chute with a boom truck the hardware was removed from flanges to split the chute at a joint. After removing the hardware a hammer was used to strike the chute to dislodge it. When the chute came apart it shifted to one side breaking through a hand rail and striking the employee on the right foot.
Using a spud bar to clean out a chute on the S2, reaching in with the bar through an access panel. Bar slipped and right hand was caught between bar and the edge of the access panel. A severe avulsion of the fifth finger from the second knuckle and a small abrasion to the fourth finger on the right hand.
EE was removing a sheave from an electric motor. The pusher bolts had broken off, While ee hit the sheave with a hammer, He misjudged the placement of his left hand and got his left thumb struck between the sheave and the hammer. The Dr. made a hole in his thumb nail to relieve the pressure.
A missing person suffered a self inflicted gunshot wound to the head. The body was removed by the coroners office from the woods located at the back of the mine property. This individual has never been employed by the mine, nor the company.
Employee was getting silo unplugged with a pry bar when the material let loose and smashed and cut his finger between the pry bar and metal on side of conveyor.
EE was making a repair to the belting on C-1 conveyor. He was using a utility knife to cut a piece of belting to use as a patch. He was kneeling on the conveyor catwalk and cutting the belt. As he was cutting the belt, the knife slipped and he struck his left thigh with the knife causing a laceration approximately 1 inch long by 3/8 inch deep.
Employees were adjusting a blow bar in a H.S.I. crusher. A 3lb. sledge type hammer was being used to knock the bar retaining wedges loose so the bar could be slid into position on the rotor. When an employee struck a wedge, a piece of metal broke off the hammer face and struck the employee in the right forearem. This caused a deep cut to the employee's arm.
Hand was in pinch point between driving bar and blowbar while being struck by sledge hammer. Impairment: deep bruise and accumulation of blood under the skin on left little finger.
Employee was reinstalling a lid on a horizonal crusher after repairs. Employee was hitting the lid with a hammer to align the bolt holes, employee missed the edge of the lid with the hammer, swung back where his other hand was resting, striking the little finger of his left hand.
REMOVING A TAT 5 GEAR REDUCER FROM CONVEYOR. HE WAS USING A 16" PRY BAR TO HELP REMOVE REDUCER. REDUCER BROKE LOOSE, LETTING PRY BAR TO SLIP. EE FELT A POP IN AND BURNING IN LEFT SHOULDER.
Employee was replacing toggle plate in jaw crusher, releasing come-along to reseat toggle plate. Employee had his hand on chain of come-along when toggle plate popped up, tightening chain and pinching finger between chain and jaw crusher, causing a laceration on left middle finger.
EE WAS PRYING ON AN IDLER WITH A SPUD BAR. SPUD BAR SLIPPED, CAUSING EE TO HIT HIS LEFT INDEX FINGER ON THE IDLER.

EE WAS CLEANING SPILLAGE FROM A WALKWAY ALONG THE CRUSHER. SOME OF THE MATERIAL WAS FROZEN, SO A SPUD BAR WAS BEING USED TO LOOSEN IT. ONE EE WAS USING SPUD BAR & 2ND EE WAS SHOVELING IT OFF T HE WALKWAY. THE SPUD BAR BOUNCED OFF A STONE AND THE OTHER END OF THE BAR STRUCK THE EE IN THE FOREHEAD CAUSING LACERATION TO HIS FOREHEAD ABOVE THE RIGHT EYE. HE RECEIVED 3 SUTURES TO CLOSE
PULLING CROSS SHAFT OUT OF SCREEN HOUSING. LOWERING SHAFT FROM ONE CATWALK TO ANOTHER USING SLINGS TO LOWER SHAFT. WHEN SHAFT WAS ABOUT 2' FROM CATWALK, EE HAD HOLD OF SLING TO STEADY IT. SLIN G SLIPPED OFF OF SHAFT & LANDED ON HIS LEFT FOOT. SAID HE WAS FINE & DID NOT NEED A DOCTOR. BUT WENT LATER TO ER FOR XRAYS. NOTHING WAS BROKE.
EE WAS REPLACING 1'X1' URETHANE PANELS IN SCREENDECK OF 6'X16 TRIMMING EDGE OFF PANEL, WHEN KNIFE SLIPPED & CAUSED INJURY
A ROCK HAD BECOME LODGED IN A CRUSHER CHUTE & EE WAS TRYING TO DISLODGE IT WITH A PRY BAR. THE ROCK CAME LOOSE BUT THIS CAUSED HIS RIGHT FOREARM TO STRIKE THE CHUTE.
AFTER LOCKING OUT CONVEYOR BELT, EE BEGAN CLEANING SNOW FROM THE RETURN SIDE OF BELT WITH A SHOVEL. EE STATES THAT SHOVEL GOT TANGLED IN CONVEYOR FRAME. WHILE TRYING TO FREE IT, EE LOST HIS BA LANCE, STEPPED BACK ONE STEP AND FELL OFF OF CATWALK.
EE TRIED TO CUT A WIRE TIE WITH A KNIFE, IT SLIPPED OFF & HE CUT HIS RIGHT HAND BEHIND HIS THUMB.
EE WAS ATTEMPTING TO CUT A PIECE OF NEOPRENE RUBBER OFF SO THAT IT WOULD FIT INTO PLACE ON A SCREEN DECK AND THE UTILITY KNIFE HE WAS USING STRUCK HIM IN THE RT. THIGH AREA. THE RESULT FROM TH IS ACCIDENT WAS 5 STITCHES TO HIS RT. THIGH.
EE WAS ATTEMPTED TO DISLODGE THEY BLOCKS BY RPYING WITH A BAR ON 100 TON BIN WHEN PRY BSRT SLIPPED AND SMASHED TWO FINGERS ON LEFT HAND.
