Showing all 2At the time of the incident employee was removing cutting edges from the bucket of a CAT 992C loader. To prevent the square shoulder of the bolt from slipping out of its locking lug while removing the nut he would hold the bolt in place while engaging the impact wrench. When the impact wrench was engaged the bolt slipped out of its lug and spun, cutting his hand.
