Employee was trimming a newly installed grease line for a bearing installed on a tail pulley. When employee cut through the grease line ee cut left index finger. EE was seen at the occupational on 06/30/2017 where ee received butterfly bandages. EE was released to restricted duty on 06/30/2017 with a follow up appt. on 07/06/2017.

Showing all 6Employee reported that he was removing an old rubber grease line to install a new longer one. He stabbed his left wrist with his pocket knife when his pocket knife slipped while cutting the old grease line.
Employee was doing repair work in sandscrew. The employee was finishing the job and throwing a 3/8" lifting chain out of the tub, over the side, to the ground. As he released the chain, the grab hook caught on his right glove and pulled his arm and strained his right shoulder.
The employee was cutting a frozen rubber hose with a utility knife. The kniffed slipped and cut his finger.
EE WAS TRYING TO ALIGN THE THREADS ON A CRUSHER CAP NUT. HE WAS PULLING AND ANOTHER EMPLOYEE WAS PUSHING ON A PRY BAR. THE PRY BAR SLIPPED OFF WHEN THE CRUSHER CAP MOVED. EE'S FINGER WAS CAUGHT BETWEEN THE CRUSHER CAP AND TORCH RING ON CRUSHER. EE CONTINUED WORKING FOR A COUPLE OF DAYS. WENT TO M.D. WHEN FINGER PAIN CONTINUED. FRACTURE TO FINGER.
EE WAS SWINGING A SLEDGE HAMMER & HIS RIGHT ARM BECAME SORE.
