While doing maintenance to one of the pickup trucks, he hit the shaft with a hammer and a chip broke off the shaft, leaving a 1/2" laceration on his right cheek on his face and a small piece of metal got imbedded.

EMPLOYEE WAS PERFORMING SCHEDULED CLEAN UP UNDER A CONVEYER. HE WAS ON HIS KNEES REACHING MATERIAL WITH A SHOVEL WHEN HE EXPERIENCED PAIN IN HIS RIGHT SHOULDER AND NECK AREA. THE PAIN PERSISTE D AND HE WAS DIAGNOSED WITH A RIGHT SHOULDER/ CERVICAL SPRAIN.

EMPLOYEE WAS WORKING ON LOOSENING THE BRAKES ON A YARD TRUCK. HE WAS STRIKING A PIN WITH A HAMMER AND MISSED THE PIN HITTING HIS LEFT THUMB.
