QUESTION 5A- UNDERGROUND LOCATION IS ON BELT LINE. MINER WAS MUCKING BELT LINE WHEN A PIECE OF COAL FLEW UP AND HIT HIM IN LEFT EYE. SAW THE DOCTOR AND MEDICATED IT FOR A COUPLE OF DAYS. ENDED WITH NO PROBLEM.

EE WAS USING A BAR TO MOVE STRAPS, WHEN A STRAP SLIPPED CAUSING BAR TO HIT HIM ON LEG. THE IMPACT WAS TO THE SHIN AND CAUSED IMMEDIATE BRUISING AND PUT A DENT INTO LEG. WENT TO LOCAL HOSPITAL, X-RAYS SHOWED NO BREAK. THIS OCCURED ON THURSDAY. EE TOOK OFF REST OF WEEK AND RETURNED TO WORK MONDAY 7-22-02 WITH NO RESTRICTIONS.
