DRILL OPERATOR WAS REPAIRING THE DRILL-REMOVING A HOUSE JACK. HE WAS USING A SMALL HAMMER TO DRIVE OUT A PIN WHEN THE HAMMER HEAD BROKE AND A PIECE OF THE FLYING METAL STRUCK HIM IN THE RIGHT FOREARM CAUSING A PUNCTURE WOUND AND LACERATION OF UNDER FOREARM.
