EMPLOYEE WAS LOOSENING A NUT ON A EYE BOLT THAT WAS TIGHT. AFTER HE LOOSENED THE NUT HE FELT A PAIN IN HIS NECK AND LEFT SHOULDER.

EMPL WAS ASSISTING IN MAINTENANCE REPAIR OPERATION THAT OF CHANGING WEAR PLATES & CAPS ON BUCKET OF DRAGLINE. AS HE WAS SLEDGING PIN OUT OF BUCKET TOOTH SLIVER OF STEEL CAME OFF PIN & LODGED I N FACE JUST ABOVE UPPER RIGHT SIDE OF LIP.
EMPLOYEE WAS REPLACING TWO WORN RAILS OF RAILROAD TRACK AT LOADOUT FACILITY OF PINEY T IPPLE IN ORDER TO REMOVE WORN TRACK(RAILS) THE SPIKES HOLDING THE RAILS IN PLACE HAVE TO BE REMOVED THIS IS DONE WITH THE USE OF A TOOL CALLED A NAIL BARDURING THE COURSE OF REMOVING THE SPIKES THE EMPLOYEE BEGAN TO EXPERIENCE PAIN IN HIS LOWER BACK & LEGS PAIN INCREASE OVER THE NEXT FEW DAYS & H
EMPLOYEE WAS ASSISTING IN REPLACING WORN TEETH ON THE DRAGLINE BUCKET.HE WAS HOLDING THE B & O AS A FELLOW EMPLOYEE WAS STRIKING THE B & O WITH A SLEDGE.THE B & O HANDLE BROKE OFF AND THE SLDE GE STRUCK THE WEAR PLATE,CAUSING A SPRAWL FROM THE WEAR PLATE TO STRIKE EMPLOYEE IN THE RIGHT CALF.

EMPLOYEE WAS ST5NDING 15-20 FEET FROM WHERE A FELLOW WORKER WAS TAPPING A BEARING ON A SHAFT WHEN ASPRAWL FROM THE PUNCH,HAMMER OR BEARING STRUCK THE EMPLOYEE ON THE RIGHT SIDE OF THE CHEST.
INJURED WAS AT 6ECKWITH'S FACILITY CHAINING DOWN D-10 ENGINE. AS HE WAS TIGHTENING THE CHAIN BINDER HE FELT A SHARP PAIN IN HIS LEFT LEG.
