THE INJURED WAS ASSISTING IN CARRYING OUT THE CUTTERS BITS ON THE CUTTER HEAD OF THE TSM MINER BY HOLDING THE BIT TOOL FOR ALFRED RUSSEL TO HIT IT WITH A HAMMER. RUSSELL SWUNG THE HAMMER HIT T HE BIT TOOL & A PIECE OF METAL FROM THE HAMMER STRUCK THE INJURED.
