INJURED WAS JACKING THE BELT TAILPIECE AND THE BAR HE WS USING KICKED BACK AT HIM WHEN THE TAIL PIECE SLIPPED. HE WAS STRUCK ON THE LEFT SIDE OF HIS NOSE CAUSING A LACERATION AND BREAKING HIS NOSE.

HE HAD SET A STIFF JACK BETWEEN THE TOP AND BOTTOM TO PULL HIS BRIDGE CONVEYOR WITH AND THE JACK SLIPPED HITTING HIS ELBOW WITH A GLANCING BLOW.

THE EMPLOYEE WAS HAMMERING KEY STEEL INTO A MOTOR SHAFT USING ANOTHER OBJECT FOR DRIVING (E.G. CHISEL). THE DRIVING OBJECT SPLINTERED AND SLIVERS OF THE METAL BECAME DETACHED SO THAT THEY ENTE RED THE EMPLOYEE'S HAND WHICH WAS HOLDING THE DRIVING OBJECT.
