EMPLOYEE WAS PERFORMING MAINTENANCE ON THE ROOF BOLTER. HE WAS ATTMEPTING TO LOOSEN A BOLT ON THE ROOF BOLTING MACHINE. WHEN HE APPLIED FORCE TOLOOSEN THE BOLT HIS RATCHET SLIPPED OFF THE BOLT HEAD CAUSING HIM TO HIT HIS SHOULDER ON THE MINE ROOF.
