Employee was using a utility knife to cut a cardboard box. Employee lacerated his left thumb with the utility knife.
Employee was bending a piece of 1/4" x 2" steel strap around a piece of pipe. He lodged one end of the strap between the pipe and a pipe hanger, and was heating the strap with a torch and hitting it with a hammer. The strap came out of the area between the pipe and pipe hanger and struck the employee in the mouth.
EE WAS STANDING NEAR THE TOP OF A 16' STEP LADDER SWINGING A 3 LB HAMMER TRYING TO LOOSEN A PVC FITTING WHEN LADDER WOBBLED CAUSING HIS BACK TO TWIST. THIS REPORT IS BEING FILED DUE TO PHYSICA L THERAPY BEING STARTED ON APRIL 10, 03 AND TO CONTINUE FOR AT LEAST TWO WEEKS.
