EE WAS WORKING ON THE HITACHI SHOVEL WITH A PAIR OF CHANNEL LOCKS. EE WAS USING THE CHANNEL LOCKS WHEN THEY SLIPPED AND HIT EE IN THE MOUTH, CAUSING THE INJURY. EE DID NOT SEEK MEDICAL ATTENTI ON UNTIL 9-28-96.
