Showing all 3Employee was changing bucket teeth on a loader. Employee used a hammer to remove old teeth. Afterwards he noticed a small laceration on his left forearm. Two days later his forearm was sore so he went to the hospital to get x-rayed. A small shard of metal was lodged in his forearm but the doctor couldn't find it and put one stitch in his forearm.
Employee was cleaning the tracks on a Komatsu excavator. Employee's hand slipped off shovel and shovel handle hit employee on left side of head. Employee went to hospital and got checked out and was released and returned to work next day.
