A mechanic was tightening a chain and device used to change out dozer cutting edges. An overhead crane was also used. He over-hoisted the crane which put too much tension on the device. The device failed and struck his wrist causing a laceration and fracture.

A mechanic was using a torque wrench on a nut. The wrench failed causing the mechanic to fall to the shop floor. As a result of the fall, his right ankle was broken.

A mechanic was cutting a section of heater hose with a razor knife. His other hand was stabilizing the hose. As he drew the knife toward himself, the blade contacted the top portion of his left hand above his thumb causing a two-inch laceration. The mechanic received sutures to close the wound and returned to full duty on the same shift.

An employee was using a six foot pry bar when the bar slipped and hit him in the lower lip and jaw. He received a laceration to the inside lower portion of his lower lip. Sutures were required to close the wound. The employee was released to return to full duty that same day.

A mechanic was attempting to move a retainer bolt from a hardnose pivot pin. The socket slipped off the bolt head and his face came in contact with the stabilizer bar. This caused a laceration to his chin and a chipped/cracked tooth. The laceration required four sutures and the tooth required crowning procedures.

Employee was removing a lip shroud from a shovel bucket. A hammer was used to loosen the shroud. This caused a piece of metal from the bucket to strike the employee between the eyes causing a laceration.

Mechanic was loosening a hydralic hose fitting on a Caterpillar 16G blade. Using a 1/2 inch ratchet, six inch extension, and a crows foot open end wrench. When the hydralic fitting came loose. It caused the ratchet to come free from the extension. Which struck the employee in the left lateral tooth causing a break in the tooth.

Employee was using a chipping hammer to remove a stuck bolt. He struck the chipping hammer with another hammer and a chip was propelled into the employee's left hand, dorsal side between the second and third metacarpal bones.

While lifting a second intermediate propel shaft, the chain sling being used broke striking the employee on the forehead. The emplyee was wearing safety glasses and a hard hat at the time of the incident. The wound required sutures to close.

While removing a hydraulic cylinder from the boom of a coal unloader, a come-a-long was used to secure the cylinder to the boom as the cylinder pins were removed. After the pins were removed, the cylinder unexpectedly moved striking the employee on the left cheek. The employee received sutures to close the wound.

EMPLOYEE WAS REMOVING SHEATHING FROM A PIECE OF 1" CABLE WITH A UTILITY KNIFE AND CUT THE MIDDLE FINGER OF HIS LEFT HAND REQUIRING 5 SUTURES. HE CAME BACK TO WORK THE SAME SHIFT WITH A FULL DR S. RELEASE.

EE WAS USING A RATCHET AND SOCKET, TURNING THE ENGINE OVER IN NET 590. WHILE PULLING THE RATCHET, HE DROPPED SOMETHING IN HIS RIGHT WRIST. IT STARTED TO SWELL. HE WAS SEEN BY A DOCTOR. HE WAS DIAGNOSED WITH A SPRAINED WRIST.

EE WAS USING A RATCHET TO TIGHTEN A CLAMP FOR THE EXHAUST ELBOW INSIDE THE INGIME COMPARTMENT. WHILE TURNING THE RATCHET, 1 IS FINGER GOT TOO NOSE THE METAL EDGE AND HE REC'D LACERATION, REQUI RING 4 SUTURES.

EE WAS HOLDING A PIN DRIVER ON A RIPPER POINT ON A TRACK DOZER. THE DOZER OPERATOR WAS SWINGING THE SLEDGE HAMMER. ON THE SECOND HIT, A PIECE OF THE DRIVER BROKE OFF, HITTING EE ON THE RIGHT S IDE OFHIS NOSE. THE PIECE OF METAL WAS REMOVED & HE RECEIVED 6 SUTURES.

EE HAD THE LOAD HE WAS WORKING ON SUSPENDED ABOUT 2" OFF THE CONCRETE FLOOR. THE RIGGING SLIPPED JUST ENOUGH TO CAUSE THE LOAD TO COME DOWN ON HIS FINGER WHICH WAS UNDERNEATH THE FRAME AND FRA CTURED THE TIP OF IT WHILE SMASHING THE OUTER EDGE.

EE WAS TRYING TO BREAK A PLUG OUT OF A "Y" STRAINER WITH A BREAKER BAR & FELT A POP IN HIS RIGHTELBOW. HE DIDN'T FEEL ANY FURTHER PAIN SO HE FINISHED HIS SHIFT. THE NEXT MORNING HE HAD SWELLIN G IN HIS ELBOW. HE WENT TO THE DOCTOR WHO DIAGNOSED A HAIRLINE FRACTURE.

EE WAS BENDING DOWN TO PUT A SPACER IN THE LEFT SIDE OF A NOSE CONE ON A HAUL TRUCK. AN ANCHOR CHAIN FAILED AND A 3 TON COME-ALONG SWUNG DOWN AND THE CHAIN OR PART OF THE COME-ALONG STRUCK HIM ON THE LEFT SIDE OF HIS HEAD CAUSING A LACERATION WHICH REQUIRED SUTURES.
