Dropping miner cable. Reached to cut rope hanger with knife in hand. Cable hanger broke and cable came down hitting left hand with knife in it, pushed knife into right index finger. Received 6 stitches

employee was removing a universal joint from a drive shaft. while driving the old caps out with a hammer when a piece of the end of the hammer broke off and flew into wrist.

EE was installing a fish plate to rails and was located behind co-worker, who was using a pole axe to turn the tabs on a track tie. The axe slipped from his hand and struck EE above the eye. He was given first aid and chose to work the remainder of his shift. EE went to his physician and received four sutures and a head scan. He was released back to work. No days were lost.

Employee was knocking track ties on track rail. Rail was blocked up. Employee stuck rail with hammer causing rail to fall on top of right foot. Employee was transported to SMH by ambulance. X-rays reveled no fracture

The EE alleged that while he was putting a ripper jack on the LCM; he received a strain to his shoulder. An investigation was conducted and the operator determined that a workplace injury did not occur. This report is to abate citation 8154333. The operator does wish to contest this citation.

Employee holding a punch to drive out a stuck bolt when a small piece of metal off the punch broke off when struck by a hammer. The piece of metal hit right forearm causing a small laceration and imbedded in arm. No stitches at time of injury but due to swelling in arm doctors decided to remove metal on 08/17/11, at this time five stitches was required.

Unloading rails from car, rails become lodged on end of car, used chain hoist and clevis attached to a roof plate, eye in roof plate broke causing rails to fall, one of the rails hit employee on top of foot resulting in a fracture to right foot.

Using a hammer on a tire hub, a piece of metal broke off and was embedded in the employee's little finger on right hand. He was taken to Summersville Regional Hospital and had the metal removed. The incision required 2 stitches.

EE stated that he was hitting a metal bar with a sledge hammer while putting a bushing into the cutter on miner and a metal chip hit his forehead. EE went to the doctor on 4/29/10 and he removed a small piece of metal and the EE received 7 stitches.

Member was prying up on a shuttle car tire using a slate bar. The slate bar slipped and the shuttle car tire fell on members left leg. Note: EE worked until 5/22/09 and completed his shift.

On 1/7/09 EE was in the process of making a belt splice and while using a knife to cut the corner off the belt, EE cut his little finger on his right hand. first aid was done and EE went for medical treatment after completing his shift.

On Nov 19, 2008, employee was in the process of changing a broken miner bit. While he was striking the insert with a sledge hammer, his left hand slipped down the bit lug resulting in him striking his left hand against the insert with the sledge hammer.

EE was using slate bar to turn a 33' long rail, he was being assisted by another EE from the opposite end of the rail. As they were turning the rail the slate bar slipped from EE's hand resulting in the slate bar hitting him on the right side of his head. 2 stitches were required to close the wound.

On 9/22/08, EE was tightening the wheel bolts on a shuttle car. While tightening one of the bolts, the socket slipped off, casing EE to lose his balance and fall on and twist his right ankle and foot. EE was treated and diagnosed with a sprain and returned to work. On 9/27/08, EE was re-examined by another doctor and diagnosed with a fracture.

Employee was hanging up fly pads. He went to hit a nail with his pick hammer. The pick hammer was turned backwards and it punctured his left hand.

On May 3, 2006, EE and #1 unit crew were on their way outside. When they got down on #6 belt, the belt 1 fire boss was wrecked and had them blocked. EE was helping put mantrip back on track. EE was bent down helping jack. When he stood up, his knee popped and he experienced sharp pain in his knee.
