HE was using a thread tapping tool. He put pressure on the wrench and the tool broke, causing him to fall forward , cutting his index finger on the tool.( he received stitches.)
He was working to free up chain on chain hoist ( come-a-long ) One end was hooked on I-Beam, while pulling on chain, it came loose and stuck him on the upper lip. ( received stitches )

HE WAS TRYING TO DISLODGE DEBRIS IN A PINNER STEEL, HE WAS USING A WRENCH TO FREE UP STEEL, WHEN HE HIT THE STEEL THE INSERT OF THE WRENCH CAME OUT AND STRUCK HIM.
