EE was driving a jack pin out of miner. A piece of metal came back and struck upper lip. (EE received stitches).

Employee was pulling on wrench, when the wrench came off, employee struck elbow on the fender of a Mantrip. ( a tear in tri-cep) ** started missing 7/11/2016**

HE was using a thread tapping tool. He put pressure on the wrench and the tool broke, causing him to fall forward , cutting his index finger on the tool.( he received stitches.)

He was working to free up chain on chain hoist ( come-a-long ) One end was hooked on I-Beam, while pulling on chain, it came loose and stuck him on the upper lip. ( received stitches )

He was tightening up nut on scoop. A helper was holding backup with another wrench on top. The helpers wrench slipped and struck him in mouth. (He received stitches )

He was making spice on pinner cable, when his knife blade went through cut, cutting bottom of left thumb. (He received stitches)

While taking about a belt splice, he was hitting a hammer against metal bolt cutters and a piece of medal came off hammer and lodged in the bridge of his nose. Dr. had to remove sliver of metal from nose.

While hitting chisel with hammer, the hammer slipped off smashing hand between the two. ***Stitches***

While cutting a vertical belt pocket with a utility knife, the blade broke causing his hand to slip, cutting the top of his hand. ***5 Stitches***

While hitting a broken bit with a chisel and hammer, a slither of metal broke off and entered his left forearm. ***He needed 2 stitches***

Employee was using come-a-long to pull a 3-wheeler out of mud hole, chain came loose, hitting him in forehead. (He received stitches to his forehead).

Using a sledge hammer to break up a piece of limestone, the rock broke and the hammer slipped causing left middle finger to strike rock. ** 6 stitches **

He was helping mechanic make a car splice and his belt knife slipped cutting his left forearm. *** stitches ***

He was helping the mechanic splice the bolter cable and slipped with belt knife, cutting his left thumb. **4 stitches**

He was pulling pins out of belt splice with a hammer and hit two hammers together. A sliver of metal from the hammer struck him in left arm. **Employee was given 7 stitches.**

He was splicing miner cable with belt knife, hand slipped and cut finger on left hand. *** 2 stitches ***

While installing lid of distribution box with a bar, the bar slipped out of the hole striking employee in mouth. *(stitches in lip)*

While helping make a car splice he cut his left finger with belt knife. Resulting in 5 stitches.

while holding a feeder cable for another mechanic while he cut it with belt knife, belt knife slipped and cut the knuckle on his left hand , / he had to have 3 stitches.

While cutting on belt, knife slipped and cut his right leg. He had 9 stitches to close wound.

He was cutting conveyor belt with a belt knife, when he accidently cut left forearm , needing two stitches.

HE WAS STRIKING HAMMER AGAINST ANOTHER HAMMER, METAL FELL INTO ARM.

HE WAS CUTTING THE BANDS ON THE STRUCTURE PILE AND THE BAND BROKE CAUSING STRUCTURE TO ROLL ON ANKLE.

While splicing cable, pushed to hard on utility knife slipped cutting index finger.

He was using a ratchet to loosen bolt, when the ratchet slipped and spun around striking left finger.

While tightening blades on pipe fusing machine, allen wrench slipped out of hole and finger hit cutting blade.

HE WAS CUTTING PIECE OF CURTAIN AND CUT INDEX FINGER

HE WAS CUTTING ON CABLE, WHEN HIS KNIFE SLIPPED, CUTTING LEFT HAND.

HE WAS PULLING ON A WRENCH, THE WRENCH SLIPPED OFF AND HE FELL BACKWARD, HIS FOOT WAS STUCK IN THE MUD AND HIS KNEE POPPED.

He was shoveling on beltline, when he went to get a scoop full of coal, the shovel slipped out of his hands, causing him to lunge forward causing lower back pain.

EMPLOYEE WAS HELPING SPLICE A CAR CABLE, HE WAS CUTTING OFF OUTER JACKETS, WHEN KNIFE SLIPPED OUT CABLE CUTTING THUMB.

HE WAS TIGHTENING UP BOLTS ON FRONT OF RIDE, WHEN HE HURT HIS SHOLDER AND PULLED MUSCLES IN BACK. *STARTED MISSING 03/29/06*

HE WAS CUTTTING BELT WITH UTILITY KNIFE, HE LAID THE KNIFE DOWN ON GROUND AND SAT DOWN ON TOP OF IT, CUTTING THE BACK OF LEG.

HE WAS CUTTING METAL BAND OFF A BUNDLE OF ROOF BOLTS, AFTER CUTTING THE BAND, IT FLEW BACK STRIKING HIS HAND AND CUTTING HIS RING FINGER.

EE WAS CUTTING A PIECE OF CURTAIN WHILE ON A LADDER, HE LOST HIS BALANCE AND TRIED TO GRAB THE LADDER RESULTING IN CUTTING HIS LEFT HAND WITH BELT KNIFE.

HE WAS TRIMMING CABLE WITH KNIFE, WHEN THE KNIFE SLIPPED AND CUT HIS LEFT INDEX FINGER .

Employee was trimming edge of a belt pocket on vertical belt with belt knife when the knife slipped through & cut his left hand, which needed 7 stitches.

HE WAS CUTTING BELT, WHEN KNIFE SLIPPED OFF BELT CUTTING KNEE.

HE WAS CUTTING RUBBER OFF BUCKETS WHEN THE BELT KNIFE SLIPPED OFF BELT, CUTTING HIS FOREARM.

EE WAS HELPING BUST UP A ROCK IN THE FEEDER, WHEN A PIECE OF ROCK STRUCK HIM IN THE NOSE.

The tail piece was being broken down so it could be moved. EE was walking by end of tail piece & jack pipe came loose from roof, falling and striking EE on left arm.

Employee was prying a hitch apart on two trailers and possibly pulled a muscle in her lower back or hip area.

HE WAS TRYING TO DISLODGE DEBRIS IN A PINNER STEEL, HE WAS USING A WRENCH TO FREE UP STEEL, WHEN HE HIT THE STEEL THE INSERT OF THE WRENCH CAME OUT AND STRUCK HIM.

He was splicing car cable, when he cut his left hand with a belt knife.

HE WAS NAILING UP NAIL, WHEN HE SMASHED HIS LEFT INDEX FINGER.

HE WAS DRIVING PAN PINS OUT WITH SLEDGE, WHEN HE FELT SOMETHING PULL IN HIS MIDDLE TO UPPER BACK.

HIS LT. THUMB WAS SMASHED WITH A SLEDGE HAMMER WHILE HE WAS CHANGING RETURN FILTER PANELS.

REMOVING BOLTS FROM FORMS ON "B" SIDE OF WALL. ASKED TOPMAN TO BUMP UP ON WINCHES. DUE TO MISCOMMUNICATION WINCHES WERE RUN UP UNTIL PAD EYES BROKE & HIT EMPLOYEE.

HE WAS STANDING BACK & WAITING WHILE TWO OTHER EMPLOYEE'S HAMMERED A CHAIN TOGETHER. WHEN A SLIVER OF CONVEYOR CHAIN STRUCK HIM IN THE NECK.
