INJURED WAS STARTING TO MAKE A LIFT WITH A RATCHET TYPE COMEALONG. INJURED HOOKED THE RATCHET END TO THE BOTTOM FLANGE OF THE BEAM, LOCATED AT FOOT OVERHEAD. WHEN HE STARTED TAKING UP THE SLAC K CHAIN, THE HOOK SHIFTED OFF THE BEAM, STRIKING& GLANCING INJURED'S LEFT FOREHEAD AND HARD HAT. THE HOOK HAD A SAFETY LATCH OPERABLE, BUT COULD NOT BE LATCHED DUE TO THE HOOK UP PLACEMENT. I
