Employee was removing nozzle from the end of a fire hose. It was too tight to remove by hand so the employee held it with one hand and hit the coupling with a hammer. On the third swing he missed the coupling and struck his left hand resulting in a fracture.

Employee was changing hydraulic hose on the roof bolter when the wrench slipped causing him to strike his finger against the roof bolting machine resulting in a fracture.

EE WAS WORKING ON A TELEPHONE LINE, AND CUT HIS LEFT HAND.THIS REQUIRED SIX STITCHES.

EE was in the process of jacking a derailed track flatcar back on the track, the jack bar slipped, causing EE to twist his lower back and left hip.

EE was trying to pry scoop batteries out of battery tray with a slate bar. Slate bar slipped causing ee to fall backwards and twist his right knee. Weather conditions were raining and wet.

EE was putting a wrecked flat supply car back on the track, he was aligning the trucks of the flat car on the rail with a slate bar, when the car shifted and mashed his finger between the slate bar and rail.

EE was taking old bits out of the cutter drums, knocking them out with a bit hammer and mashed his finger

Co-worker was attempting to put the mantrip back on the rails. A jack was set on crib blocks. While pushing the jack bar down, the jack gave out and hit employee in the face. He was cut in the face and lost 5 teeth.

Employee was knocking out a brattice when a piece of cinder block hit him on the left arm causing a laceration to the left arm.

EE was maiking a belt splice and hit the index finger with a hammer.

Employee was using channel locks to break the metal bands holding 3x8"x4x4'angle together on the bed of the boom truck when one of his bands broke hitting his right wrist causing a laceration.

EMPLOYEE WAS WORKING NEAR ROTARY BREAKER USING SLEDGE HAMMER (10#). HAMMER HIT ROCK AND BROKE HANDLE. STEEL HEAD OF HAMMER CAME BACK AND HIT EMPLOYEE ON RIGHT CHEEKBONE. LACERATION REQUIRED 3 STITCHES.

INJURED WAS REPLACING BITS ON A CONTINUOUS MINER MACHINE. HE WAS USING A PICK HAMMER TO REMOVE A STUCK BIT, WHEN A PIECE OF METAL FROM THE BIT PENETRATED HIS RIGHT THUMB.

THE INJURED AND A CO-WORKER WERE ATTEMPTING TO REMOVE A BURST HYDRAULIC HOSE ON THE RIGHT SIDE CONTINUOUS MINER. TO BREAK THE FITTING LOOSE, THE CO-WORKER WAS USING TWO PICK HAMMERS. A SMALL PIECE OF STEEL THE APPROX. SIZE OF A BB BROKE OFF OF THE FACE OF ORE OF THE HAMMER, STRIKING THE INJURED IN THE LT. FOREARM AND PENETRATING INTO THE TISSUE.

EE WAS OPENING A BAG OF ROCKDUST WITH A UTILITY KNIFE. HE CUT HIS LEFT HAND.

EMPLOYEE WAS MAKING A SPLICE IN A 1 1/2" WATER LINE AND WAS PUTTING A BAND ON THE COUPLING WITH A BANDING TOOL AND HAMMER. THE HAMMER CHIPPED OFF AND WENT INTO HIS RIGHT INDEX FINGER. HE REQUI RED 4 STITCHES WHEN REMOVED.

EE WAS CUTTING OPEN A CARDBOARD BOX & CUT THE CORNER WHEN IT JERKED CUT HIS RIGHT ARM. RECEIVED 2 STITCHES AT CLINH VALLEY MEDICAL CENTER

EE WAS CUTTING THE #7 BELT INTO WITH A BOX KNIFE. WHEN THE BELT WAS CUT INTO IT JERKED AND CAUSED EE TO CUT HIS INDEX FINGER ON HIS LEFT HAND.

EE WAS ATTEMPTING TO RERAIL AN 11 TON LOCOMOTIVE WITH A RAILROAD JACK WHEN THE JACK BAR SLIPPED, HITTING EE IN THE LIP & CHEEK.

EE WAS HELPING PUT SUPPLY CAR BACK ON TRACK. HE WAS USING A JACK. HIS FINGER GOT CAUGHT BETWEEN JACK AND CAR. THE END OF HIS INDEX FINGER WAS SMASHED AND REQUIRED STITCHES.
