Showing all 37After an employee hooked a come-along up to a drag, EE stood up and felt a sharp pain in EE's left hip. The employee was taken off work by a doctor's restrictions on 10/06/2025.
Employee was changing a BO top roller. As EE was working a come-along to raise the belt, EE felt a sharp pain in EE's right shoulder. The employee continued to work until 04-23-2026 when EE went off for surgery.
I.E. was changing a tire on a coal hauler. While loosening the lugs with a ratchet and pipe, the head of the ratchet slipped causing the I.E. to smash their right ring finger. The finger was between the rim of the tire and the pipe. I.E. received a fracture in the end of the ring finger.
I.E. was cutting cable back to prepare it to be reentered into a cat head. As their knife went through the cable jacket, the blade struck their left hand causing a laceration in between their thumb and index finger that required 4 stitches.
EE was removing drill steels from the roof. The hole was not straight and the drill wrench had tension on it. When the steels came free the wrench came back striking the EE on the right hand. EE was seen by a doctor on 11-11-2022 and a fracture in the hand was discovered. EE is still working on restricted duty. "IE has been taken off by an orthopedic doctor as of 11/17/22."
I.E. was helping pour concrete for a new belt drive setup. As they were pulling concrete with a concrete rake, they felt a pop and burning sensation in their right elbow. I.E. is still working under restricted duty.
I.E. was cutting a piece of belting cover to access a water line fitting in the throat of the miner. As they were cutting the belting the knife slipped and caused a laceration to the tip of their left index finger. The laceration required stitches and the employee has not missed any work.
I.E. was hitting a pacer pump in a water tank with a hammer to clean the coal out of the pump. While swinging a hammer they felt a burn in both their shoulders. I.E. has continued to work until a doctor took them off for surgery on 8-23-2022.
I.E. was attempting to unplug the reject chute at the breaker station which a prybar. As they were prying rock, some broke loose causing them to smash their left pinky finger between the bar and a surface. This resulted in stitches to the finger. I.E. has not missed any work.
I.E. was recovering curtains from an old set of rooms. As the I.E. was pulling spads out of a 2x4 with a spad puller, it slipped from their grip and left a laceration on their left hand requiring stitches. I.E. has not missed any work due to the injury.
IE was using sledge hammer while making a repair on a roofbolter. As IE went to swing hammer, felt a pop in left shoulder area. IE has not missed any work. Reporting due to add'l treatment in form of pain shot given 02/21/20. Amendment: I.E is scheduled for surgery 5/12/20 & will be off work till released. Hasn't missed any work between inj date & surgery date.
Using spad tool to take down pull thru curtains and pinched finger between bolt head and tool. EE finger required sutures.
While scaling down roof material, employee struck their right elbow on a wrench, chipping a bone in the elbow . It did not require any medical treatment.
Employee was cutting a hole in a belt dribble chute when something got in the corner of left eye. EE was prescribed prescription eye drops.
Employee was holding a hammer while another employee attempted to strike hammer to drive a gear coupling off. The 2nd employee missed the hammer head and struck the handle causing the injured employee's hand to strike the motor shaft. The employee received stitches to left middle finger.
Employee was cutting bands on screen wire. Band came back and cut left hand. The reason for the delay in reporting was completion of the accident investigation. EE had micro surgery on February 4, 2016 and became reportable at that time. EE did not miss any work. EE is currently on restricted duty.
Employee was removing tap from hole. The wrench slipped and he cut his thumb on the bit. His thumb required 3 sutures.
Employee was using a four way lug wrench to tighten lug bolts on wheel, as he leaned to put his weight on wrench his right ankle buckled as he twisted his body. Conditions were dry and daylight.
Tool got hung on canopy of roof bolter and bent left thumb backwards while hitting drill steel with a tool,
Contractor who works for Jones Hydroblast was clearing a chute and his pry bar struck him on the jaw.
Cleaning pan and caught his finger between the pan wall and scraper. The reason for delay in reporting was delay in x-rays and results.
Employee was cutting the belt to make a splice. The blade on the belt knife broke and the knife was pulled into his left leg causing a 1 1/2 to 2" laceration
Employee was driving a spad into the rib. As he swung the hammer, he felt something "pop" in his right arm.
Employee was putting up sites in #2 entry of unit 3. He was driving a plug into the roof with a hammer when he felt something pop in his right hand and wrist. He started missing work on 5/31/13. This report was delayed waiting upon medical reports.
Mechanic was changing out the canopy lift cylinder on a roof bolter. He chained the canopy to the roof on a chain hanger and cut the pins out of the cylinder, they applied heat to the cylinder eye and applied down pressure with the boom of the bolter and then hit it with a sledge hammer. The cylinder broke loose and the miners foot was under the drill pot of the bolter.
Employee was installing a center pin in coal hauler 141. He struck the pin with a sledge hammer, when he did a small sliver of the hammer broke off and embedded in his right cheek.
Employee was swinging the panel for the miner back in, using a pry bar for leverage. The bar slipped causing the employee to fall forward hitting his head on the top of the miner. The employee had a 1 1/2 inch cut above is right eye. He received 5 stiches.
Employee was changing a bit cone on the miner. He was trying to put the snap ring on the cone, while he was having another employee hit the cone with a sledge hammer. He got his hand in between the top of the cone and the sledge hammer, smashing his left ring finger.
miner was struck in the forehead while attempting to removing a belt splice pin using a claw hammer, he hit the claw hammer with another hammer causing a piece of metal to chip off and hit him in the forehead just above his safety glasses.
Employee was slicing an apple with a razor knife and the knife slipped cutting the palm of his left hand. stitches were applied.
Repairman was repairing a feeder conveyor chain. He was driving the keeper back on the pin when a piece of the tool he was using as a punch flew off and went into his left lower thigh. He recieved 3 stitches and has not missed any work due to this accident. The piece of steel that went onto his leg was approx. 1/2" X 1/4" X 1/4".
Was unplugging chute in prep plant using a 3/8" rod, the rod slipped out of his hand. He reached out for it and at that time he felt pain in his lower back. Started missing work on 1/11/11.
Employee was driving a pin out of a segement roller on a Cat Dozer with a hammer. A piece broke off of the pin hitting him in the forehead about 1" above the right eye.
REINSTALLING SKIRTBOARD RUBBER ON CONVEYOR, USING SCREWDRIVER TO PUSH SKIRTBOARD RUBBER IN WITH RIGHT HAND. SCREWDRIVER SLIPPED & IMPALED LEFT PALM, REQUIRING X-RAY, CLEANSING & TWO STITCHES.
EE WAS TRYING TO REMOVE DEBRIS WHICH WAS PLUGGING PLANT. USING PIPE AS PRY BAR, FINGER WAS PINCHED BETWEEN PIPE & METAL RESULTING IN FRACTURE & LACERATION TO LEFT RING FINGER TIP.
EE WAS CHIPPING FROZEN COAL IN HOPPER, TRYING TO UNPLUG FREE UP. BAR WHICH HE WAS USING BROKE THROUGHT THE COAL, AND RIGHT HAND, LITTLE FINGER STRUCK AGAINST SIDE OF SHIELD, RESULTING IN FRACT URE OF FINGER.
