Showing 50 of 58Employee was putting two pieces of conveyor chain together. He had a hammer hitting another hammer putting a link together. A piece of metal flew off striking employee in the right arm. The piece of metal was removed from his arm.
Employee states he was swinging a sledge hammer breaking a rock on the refuge belt. He lost control of the hammer, it slipped out of his hands, and he struck his left thumb against the edge of the conveyor cover sheet metal lacerating his right thumb requiring sutures.
Employee performed repetitive prying tasks at the rotary breaker head chute. On this day, employee states he pryed loose a rock in the head chute. Several days later, the employee states he found a lump on his groin area. It was diagnosed as an inguinal hernia. The employee is also a weight-lifter. Employee did not start missing time until June 22, 2011.
Employee states he was roof bolting, and stuck his drill steel wrench in the roof. He hit the wrench with a hammer and the wrench fell from the roof smashing his left little finger between the wrench and the drill pot. 3 sutures were required.
Employee stated that he was using a metal pry bar to pry a piece of rock off the top of the miner. As he was prying the rock slid very quickly off the machine and struck the victim's foot. Started missing after his regular 3 days off which was the 8th of Feb.
Employee was removing a conveyor (belt) cutter after cutting the belt for splice installation. As he removed the hold down the other employee was cranking the cutting blade out of the way and the injured employee got his little finger under the lip and in the way of the knife being moved to the side. The blade struck his little finger and caused a severe laceration to the tip.
Employee was trying to get an insert out of a roof bolter wrench. As he struck the wrench on the metal lip on the side of the bolter machine, he struck his left index finger between the wrench and lip. Injured had a laceration that required stitches and a fracture.
Employee stated he was helping splice a conveyor belt. They were in the process of installing the pin when it became lodged because of the splice clips not aligning up so he reached out to push the belt down when another person hit the belt with a hammer smashing his little finger.
Employee stated he had traveled with the MSHA insp. and after he left he was assigned to shovel out the drain in the diesel barn. As he shoveled it into a trash can and then emptied it into a small dumpster, he felt pain in his lower back.
Employee stated that he and another person were trying to remove a steel cover on a Cont. miner with a pry bar. The cover came loose and the bar slipped, and when it did, the cover went up then down and struck the employees thumb. This caused a fracture to his thumb nuckle. Surgery had to be preformed on the thumb.
Employee was cutting tape off of a pipe with a utility knife. The knife slipped and cut him in the hand.
Employee stated that he was making a miner cable splice and was stripping the inter leads and holding the cable with his left hand. As he started to make a cut with the knife, it came in contact with his thumb. This caused a laceration.
Employee stated that he was cutting a piece of line curtain and pulling the utility knife toward him. He was holding the curtain with his right hand and when the knife slipped the blade struck his thumb. The injured required 6 stitches.
Employee was using a belt knife to cut a piece of curtain. The knife slipped causing employee to stab himself in the right thigh requiring 3 stitches.
Employee is a repairman and was helping work on a conveyor motor. He and two others were prying on the motor with a 6' pry bar. The bar slipped and the victim felt pain in his shoulder.
Employee was helping square up a conveyor belt on the ground, for a splice. He was using a utility knife and the knife slipped and the blade struck him in the leg.
Employee was binding a load of supplies down, using a 3 in. strap with a ratchet. As he was ratcheting the strap tight he felt pain in his lower back. Did not miss any work until 02/27/2006. Going for MRI.
Employee was removing a rock that was hung in the Grizzly, using a pry bar and sledge hammer. He stated that when he got done removing the rock that he felt pain in his lower back. Employee did not miss work until 03/21/2006. He has a history of degenerative disc disease. Back surgery in 1975,1987 and 1997. MRI showed considerable degenerative problems.
Employees were using a come-along to pull a new impact slide pan into a belt tail piece. They were aligning the bolt holes up and as the pan was pulled foward, his finger was caught between two pieces of metal/ Avulsion/ pinched off finger tip.
Employee was helping a repairman repair on a Ramcar. He was using a Track Jack and as he was raising the Ramcar Stinger up, the Jack slipped out and the handle of the jack pinned the victim's hand against the roof.
Empoyee was adjusting a bottom roller using a prybar/screwdriver. as he attempted to move the roller with the bar, it slipped of the roller and it struck his thumb against framing. roller and bar were wet.
EMPLOYEE WAS HELPING WITH A CONVEYOR BELT MOVE AND WAS USING A UTILITY KNIFE TO CUT THE BELT WHEN IT SLIPPED AND HE STRUCK HIMSELF IN THE LOWER THIGH ABOVE THE KNEE.
EMPLOYEE WAS GETTING READY TO SCALE DOWN SOME ROCK FROM THE ROOF. HE HAD A SLATE BAR IN HIS HANDS AND READY TO PRY WHEN THE ROCK FELL ON THE BAR. THE BAR GLANCED OFF OF THE CANOPY AND STRUCK HIM ON THE RIGHT CHEEK.
EMPLOYEE WAS USING A PRY BAR, PRYING UP ON A PIECE OF ROCK ON TOP OF THE CONTINUOUS MINER. THE BAR SLIPPED AND HIS FINGER WAS SMASHED BETWEEN THE BAR AND MINE ROOF.
EMPLOYEE STATED THAT HE WAS USING A UTILITY KNIFE TO CUT RESIN CARTRIDGE BUNDLE WRAPPER. KNIFE SLIPPED STRIKING HIS THUMB OF THE OTHER HAND.
EE WAS CUTTING A HOLE IN A PIECE OF RUBBER BELTING USING A UTILITY KNIFE. HE WAS HOLDING THE RUBBER WITH HIS OTHER HAND. THE KNIFE SLIPPED STRIKING HIS LEFT INDEX FINGER.
EE WAS USING A HAMMER & BIT WRENCH TO CHANGE BITS ON CONT. MINER. WHEN HE STRUCK THE WRENCH A PIECE OF METAL FLEW OFF THE HAMMER STRIKING VICTIM IN UPPER LIP. 2 STITCHES.
EE WAS CUTTING A RUBBER WEAR LINER FOR PLACEMENT IN COAL SCREENS. HE WAS USING A UTILITY KNIFE TO CUT WITH. HE PLACED HIS KNEE ON THE RUBBER TO HOLD IT. CUTTING TOWARD HIS KNEE HE SLIPPED & CUT HIS KNEE.
EE WAS TIGHTENING UP A HOSE CLAMP, WHEN THE SCREWDRIVER SLIPPED OFF THE CLAMP & PUNCTURED EE'S HAND.
MECHANICS WERE WORKING ON A CONTINUOUS MINER, USING A SLEDGE HAMMER AND A DRIVING BAR. EE WALKED BESIDE THE WORK AREA. HAMMER GLANCED OFF BAR STRIKING MAN ABOVE RIGHT EYE.
EMPLOYEE WAS CUTTING HEX STEEL WITH CHOP SAW.
EE WAS HELPING RESET THE UNIT TAILPIECE WHEN HE GOT HIS RIGHT MIDDLE FINGER CAUGHT IN A TRACK JACK, LACERATING THE END OF HIS FINGER. HE WAS TAKEN TO THE HOSPITAL WHERE IT WAS SUTURED.
EMPLOYEE WAS RESETTING TAIL, AND WAS LEVELING IT UP WITH A JACK, AND THE JACK HANDLE SLIPPED OUT AND HIT HIM IN THE MOUTH.
INJURED STATED THAT HE WAS CUTTING THE TOP OUT OF A PLASTIC BOTTLE IN THE SHOP WHEN THE KNIFE SLIPPED AND CUT HIS INDEX FINGER ON LEFT HAND. HE WENT TO THE DR AND RECEIVED 4 STITCHES.
EE WAS BENT OVER PLASTERING WALLS AND ALLEDGEDLY STRAINED HIS LOWER BACK.
THE EE WAS CUTTING A PIECE OF PIPE WHEN THE KNIFE SLIPPED & CUT HIS LEFT HAND INDEX & MIDDLE FINGERS
EE WAS CUTTING A CABLE USING A HAMMER AND CABLE CUTTERS. THE HAMMER SLIPPED FORCING IS LEFT WRIST INTO A PIECE OF METAL ON THE CRANE HE WAS WORKING ON. THE PIECE OF METAL PUNCTURED HIS LEFT WR IST AND WAS EMBEDDED NEAR HIS BONE.

EE WAS TAKING A BELT BLOCK OFF A BELT WHEN THE CHAIN BROKE, CRUSHING HIS LEFT HAND AND FINGERS BETWEEN THE BLOCKS AND BELT ROLLER. HIS LEFT HAND MIDDLE, RING, AND LITTLE FINGERS WERE CRUSHED.
EMPLOYEE WAS XUTTING A BAND OFF A BUNDLE OF ROLLER WHEN THE BAND HIT HIS LEFT FOREARM AND ALLEGEDLY CUT IT.
THE EMPLOYEE WAS CUTTING BANDS OFF ROOF PLATES WHEN THE CHANNEL LOCK SLIPPED AND HIT HIM IN THE FOREHEAD ALLEGEDLY CUTTING HIM.
