While belting the haulage unit and tightening the tail with a set of 1/2 rails, the pin snapped on the hook of the chain causing the tail to move outby quickly, hitting the employee who was extending the jabco pull cord and was hanging it on the last j-hook just outby the tail.

Employee was helping tighten a tensioner on a belt scraper. While using the bar that comes with the scraper, it slipped and flew striking him above his left eye.
