Showing all 32IE was performing fabrication duties. the IE hit a thumb with a hammer causing a laceration requiring stitches.
Injured employee (IE) was installing a track pin using a track pin press; a metal fragment dislodged from the pin and was impaled in IE's left lower leg. Metal fragment was removed by doctor.
Technician was changing a swing cylinder on a backhoe. They screwed a bolt in the end of pin to protect the keeper threads from getting damaged while driving out the pin. While driving out the pin, the bolt sheared off and struck the technician above the right eye causing a puncture wound of about 3/8" wide.
The employee was making shim adjustments to an idler frame. The employee was using a pry bar to lift the assembly when the bar slipped striking him in the jaw. This resulted in a laceration of his chin requiring sutures.
EE was cutting material with a razor knife that was being held in his right hand. Knife slipped and blade struck EE's left hand causing a laceration between his thumb and pointer finger. Required 4 stitches to close wound.
Area was wet and somewhat muddy making vac hose slippery. Cutter/hose slipped and cut employee's finger
EE was helping pull and strip wire on unit 547. While stripping wire he slipped, cut thumb on left hand.
Employee was cutting a plastic wire tie with a utility knife. The blade struck the left hand after cutting the tie. Four sutures closed the laceration and the employee returned to work, full duty.
The collar on the Kelly bar was loosened to adjust the depth level. As ee recahed to resecure the bar, it shifted, pinching the tip of the right index finger between the bolt at the top of the bar and the top of the collar. The finger was cut resulting in six stitches.
Aligning transverse shaft bearing to bore, employee was holding 1" plate to outer race and using a sledge hammer to straighten bearing. When employee adjusted plate, the other employee was in mid-swing and hit finger of first employee.
The accident/illness was reported on 01/02/08. The accident/illness actually occurred on 11/20/07. Surgery to remove a blood clot was done on 12/14/07.
At the time of the incident employee was removing the left muffler from a 690 Tiger. As he was holding the muffler with his right hand and pressing on a prybar with his left hand the muffler suddenly broke free from the turbocharger coupler, causing his hand to be pinched between the muffler and frame and lacerating his finger.
At the time of the incident, the employee was loosening flange bolts on a transmission line. As he was pulling on the wrench, the bolt suddenly broke free, causing his elbow to swing back and strike the torque converter fill tube cap.
While driving a wooden lath into the ground, the lath splintered inti his hand causing a cut between the thumb and index finger which required stitches.
Employees changing a lift cylinder on a loader. Employee was using a 20# sledge hammer, and a piece of 1 1/2' all thread to (drive) remove an inside collar. Employee swung hammer, and missed the driver pinching his right index finger between the hammer handle and the head of driver, resulting in a finger laceration.
The ee felt a sharp pain while pulling on a chain wrench taking apart a steering cylinder con a haul truck pulled muscle under right shoulder blade.
Was cutting rubber off conveyor belt roll. Knife slipped out & hit left ring finger.
EE HIT HIS FINGER WITH SLEDGE HAMMER WHILE DRIVING CONCRETE STAKES.
An electrician was stripping the insulation off a power cable with a utility knife. The knife slipped off the cable striking and lacerating the top of his left hand. He received 5 sutures.
WHILE REMOVING "POST DRIVER" FROM POST, THE BOTTOM OF THE DRIVER CAUGHT THE RIGHT INDEX FINGER BETWEEN THE POST AND DRIVER. EE RECEIVED AN OPEN TUFT FRACTURE AND 7 STITCHES.
TECHNICIAN BENT OVER TO MARK A PIECE OF METAL WITH A MARKER AND FELT SHARP PAIN IN HIS LOWER BACK, HE RETURNED TO WORK WITH RESTRICTIONS.
TECHNICIAN WAS WORKING ON AIR LOCK IN PREP PLANT PULLING ON A SPROCKET, SLIPPED WITH PLIERS AND CUT HIS LITTLE RIGHT HAND FINGER ON GUARD. FINGER REQUIRED STITCHES.

EMPLOYEE WAS REPAIRING A LEAKING FITTING ON THE STEERING MODULE OF #634 HAUL TRUCK WHEN HE ATTMEPTED TO TIGHTEN IT UNDER PRESSURE, THE THREADS STRIPPED, FORCING THE WRENCH BACK AND STRIKING HIM IN THE FACE.
EMPLOYEE WENT TO CHECK ON CREW INSTALLING PINS IN TRACK PADS. WHEN HE WAS 5 - 6 FEET FROM THE CREW HE FELT A STING IN HIS LEFT CHEEK WHICH BEGAN TO BLEED. A SPLINTER OF METAL FROM THE EDGE OF THE PIN FLANGE SPLINTERED OFF AND ENBEDDED INTO HIS CHEEK. THIS CAUSED A 3/4" CUT THAT REQUIRED STITCHES.
EE WAS REMOVING RETAINING BOLTS & UPPER RAIL PADFROM UNDERSIDE OF THE HOUSE (REVOLVING FRAME) OFA 295B SHOVEL. HE STATED THE WRENCH SLIPPED OFF THE BOLT. HE FELL 41" OFF THE FIRST RUNG OF A 6' STEP LADDER HE WAS USING & LANDED ON THE GROUND.HE STRUCK HIS HEAD AGAINST THE MATT BOARDS BEINGUSED AS A PLATFORM TO HOLD UP THE BOOM FOOT AREAHE WAS TAKEN TO E.R. & RECEIVED SUTURES TO HEAD
CUT LEFT THUMB WHILE STRIPPING CABLE. REQUIRED STITCHES.
USING HYDRAULIC BEAD BREAKER TO DISMOUNT TIRE ON HAUL TRUCK. RT HAND PLACED BEHIND TOOL. TOOL DISLODGED UNDER PRESSURE CAUSING RIGHT HAND TO BE SMASHED ON INDEX FINGER CAUSING SMALL CUT AND PO SSIBLE FRACTURE OF INDEX FINGER.
TRUCK 3200 PARKED AT TIRE PAD FOR REMOVAL OF RRI. AS HE BEGAN TO REMOVE WHEEL NUTS WITH TORQUE WRENCH, WRENCH BINDED AND SLIPPED CAUSING HANDLE TO PIVOT AND HIT HIM IN THE HEAD.
USING HYTORQUE WRENCH TO REMOVE BOLTS FROM WHEEL. BOLT CAME LOOSE AND PINNED FINGER AGAINST WHEEL.
