EE WAS ON TOP OF A DRYER AND WAS USING A SLATE BAR TO PRY OPEN THE GATE. THE BAR SLIPPED AND HE FELL OFF DRYER ABOUT 5 FEET AND LANDED ON HIS FEET. THIS CAUSED A JAR AND TWIST TO HIS BACK. REA SON ACCIDENT REPORT IS LATE, I COULDN'T GET HIM ON THE PHONE TO EXPLAIN WHAT HAPPENED.

EE AND COWORKER WERE REMOVING A BROKEN SHEAR PIN FROM CC-4 CRUSHER USING A PORTA POWER PUSHING THE PIN AND A RATCHET AND SOCKET WITH A WELDED STUD PULLING THE FROZEN PIN. THE PIN BROKE FREE FOR ABOUT AN INCH AND EE SLIPPED STRIKING HIS HEAD INTO THE WEBBING OF THE CRUSHER CAUSING A CUT ON HIS FOREHEAD.
