Employee was running a come-along, EE thought the handle was locked in. When employee released the handle the tension brought handle back hitting employee in the safety glasses. Employee's safety glasses hit employee's eyebrow and cutting into the skin. Employee ended up getting 5 stitches.
EE WAS CUTTING FLAP ON BELT WITH RETRACTABLE KNIFE, CUT LEFT HAND NEAR BASE OF THUMB WHILE TRYING TO CUT BELT. SUSTAINED LACERATION TO HAND WITH TENDON INJURY.
While using miner puller eyelet broke and came up and hit EE in the arm.
Injured miner was working on a belt splice with two others employees setting a row of rivets, they had completed rivets and was backing away with head bowed down to avoid obstacles. One of the other employees struck a rivet and a piece of metal broke off and struck the injured miner in the left eye over the top of safety glasses. Resulting in a contusion to the eye.
Employee was installing roof support cans, the single head axe that EE was using to tighten the wedges caught a string line as EE swung the axe which caused the axe to jerk sideways striking the employee in the face. The employee received lacerations to the upper lip, lower lip, and upper gum which required stitches. One front tooth was also partially dislodged.
EMPLOYEE STATES HE WAS USING A PRY BAR TO UP-RIGHT AND ALIGN THE BALDERSON ATTACHMENT WITH THE FORKS ON A SCOOP. HE PRIED DOWN ON THE BAR, UP-RIGHTING THE ATTACHMENT. ONCE UP-RIGHTED, THE ATTACHMENT TIPPED FORWARD CAUSING THE PRY BAR TO STRIKE THE FRAME OF THE FORKS. THE PRY BAR RAPIDLY SPRANG UPWARD, STRIKING THE EMPLOYEE IN THE HEAD, RESULTING IN A LACERATION.
EMPLOYEE STATES THAT AS HE WAS CUTTING A PIECE OF WOOD WITH AN AX, HE STRUCK HIS LEFT INDEX FINGER, RESULTING IN A LACERATION REQUIRING SUTURES.
EMPLOYEE STATES HE WAS CUTTING TABS OFF A SCREEN PANEL USING A UTILITY KNIFE WHEN KNIFE SLIPPED, RESULTING IN A LACERATION TO HIS LEFT RING FINGER REQUIRING SUTURES.
EE was installing a sleeve on the wing of the raise bore head bolt, when his left 5th finger was smashed into the wedge pin, causing a laceration that required stitches.
EMPLOYEE STATES HE RELEASED BINDER HOLDING CANS ON TRAILER. TENSION ON CHAIN BECAME TOO GREAT, CAUSING BAR TO STRIKE HIM, RESULTING IN A CHIPPED TOOTH.
Employee was holding a rock dust bag above his head as a strength prank. Co-worker reached behind him and cut bag with a knife and cut his thumb.
EMPLOYEE STATES HE WAS CUTTING OUT A PIECE OF DUST LINE. THE HOSE WAS LAYING ACROSS HIS RIGHT LEG. AS HE CUT THROUGH THE HOSE, THE KNIFE TURNED AND CUT EMPLOYEE'S RIGHT LEG. EMPLOYEE LATER WENT TO THE HOSPITAL TO HAVE STAPLES; HOWEVER, HE DID NOT INFORM MANAGEMENT OF THIS UNTIL 11/06/2008.
Installing flexco splice, EE got his left finger on top of rivit plunger, when another EE hit it with a hammer.

EE STATES HE WAS ATTEMPTING TO FREE A WRENCH STUCK IN THE ROOF DRILL CHUCK. WHILE STRIKING THE WRENCH WITH ANOTHER WRENCH, HIS FINGER STRUCK AGAINST THE WRENCH RESULTING IN A LACERATION AND FR ACTURE.
EE STATES HE WAS REMOVING STEEL BANDING FROM A BUNDLE OF ROOF BOLTS. THE CUTTER SLIPPED CAUSING THE BAND TO CUT HIS LEFT FINGER.
EMPLOYEE WAS IN PROCESS OF REMOVING LAGGING FROM#3 TAIL PULLEY. THE CHAIN USED TO HOLD THE BELT WAS UNDER RATED. CHAIN BROKE AND COME-A-LONG STRUCK WORKER IN BACK OF RIGHT SHOULDER. WORKER SUS TAINED CONTUSION OF RIGHT SHOULDER.
EMPLOYEE STATES HE WAS PULLING BELTING WITH A BOBCAT. THE SCREWDRIVER USED TO HOLD THE STRAP IN PLACE WAS PROPELLED INTO THE CAB OF THE BOBCAT WHEN THE BELT RIPPED, STRIKING THE EMPLOYEE ON TH E NOSE.
