Left thumb was pinched between SCSR and slate bar while adjusting forks on fork scoop causing a simple fracture to his left thumb
Was working on putting a shuttle car conveyor chain back together. Co-worker was tightening come-a-long. The chain attached to the flight (not the come-a-long chain) broke loose striking employee in the hard hat.
Employee was pounding a wedge in on the side of a wall when the hammer missed the wedge and struck his left thumb. The employees left thumb was against a cement block when he struck it.
Employee received 6 sutures when cutting a band holding a lift of bolts. The band struck the employees left forearm causing a cut 3" long and 1/4" wide.
Employee was changing bits on longwall shearer using a bit tool when he struck his right hand with a 3 lb hammer causing his hand to be forced down on bit tool. A laceration and fracture to right pinky finger resulted from the force.
The employee was installing a connector into the end of his roof bolt drill steel. When tapping the end of the connector with a hammer, the connector flew out striking his upper lip, 4-sutures were required. The employee returned to work his next scheduled shift.
He was performing his regular duties as a mechanic when he was using a wrench & removing a blown water line from the back of the continuous miner when he felt pain in his right shoulder. He continued to work at his regular duties & continued to have pain. He was removed from work on 7/14/11 for shoulder surgery. His MRI shows severe degenerative changes.
He was using a wire rope come-along letting the screen down when the handle of the come-along struck him on the lower lip causing a laceration requiring 5 sutures.
He was breaking a piece of ice and coal that was lodged in a chute with a slate bar, the bar kicked back and the end struck him on the right cheek, causing a laceration requiring 5 sutures. He returned to work that shift with no days lost.
He was using a bit wrench while changing bits on the shearer. The wrench broke, causing him to feel pain in his lower abdomen. He continued to work at his regular duties. He went to a physician and was removed from work on 1/10/11 for an inguinal hernia.
EE was using a bow saw to cut a post when the saw jumped and the blade cut his left hand causing a 3cm laceration requiring 1 cont. suture. He returned to work without restrictions and no days were lost.
EE was using a pry-bar to separate a steel beam that was partially cut in two with an oxygen and acetylene torch. When prying with the bar, he felt a pain in his left shoulder. This foreman was taken off work on 12/14/10
He was kneeling on the floor to strip a cable when the knife slipped and cut his left leg above the knee causing a laceration requiring 4 staples.
He was using a slate bar to turn a belt pulley when the bar slipped causing him to strike his right cheek against a guarding hanger which caused a laceration requiring 7 sutures.
He was using a 3/4" ratchet and socket to break bolts free. When the bolt broke free he felt pain in his left shoulde.r Diagnosed with a strain to the left shoulder.
He was using a slate bar to lift a track tie in place and felt pain in his lower back he was diagnosed with lower back strain.
He was using a razor knife to make a hole in the skirt rubber for a bolt, when the knife slipped striking him on the right thumb, causing a laceration requiring 5 sutures.
He was using a hammer and punch to drive out a pin on a cat pad of loading machine when he struck his right index finger with the hammer causing a 2.5 centimeter laceration requiring one continous suture.
He was performing torque test on bolter with torque wrench, the torque wrench came free and struck him in the left groin area. He was removed from work for surgery to the left groin.
He was mopping the B Mains belt when the mop was caught by a return roller causing a strain to his left wrist.
He was changing bits on the continuous miner when the wrench slipped and he struck his left thumb against the wrench causing a laceration requiring 8 sutures.
He was using a razor knife to trim back to conduit on a cable, the knife slipped and he lacerated his left hand which required 2 sutures.
Cutting pipe in ditch with huskie 375 K cut-off saw with multi-purpose blade. The saw jumped up when at the bottom of the cut and struck the right side of his face.
EE was using a slate bar to knock down rock. When rock fell, it hit slate bar, knocking the bar out of his hand. He then felt something pop in his shoulder.
Employee was moving a beam with a rock bar and beam landed on foot.
Freeing gun steel with comalong - clevice broke causing chain to loosen.
He was using a knife to remove a zip tie when the knife slipped causing a laceration to the left index finger requiring 5 sutures.
Employee's left hand was caught between cable and hook while operating a chain hoist.
Injured was cutting open 1 1/4" plastic line with razor knife. Knife slipped hitting him on right leg.
He was using a pry bar to remove a top roller frame and felt pain in his right bicep. He was diagnosed with a partial bicep tear of right arm.
Injured was assembling survey tool, when pipe wrench slipped and injured came down on right leg with all of his weight and broke right fibula
He was using a ratchet to remove the bolts from the pads of a TRS cylinder. The ratchet slipped and he felt pain in his lower back.
EMPLOYEE WAS USING A HAMMER & CHISEL TO KNOCK OUT DOG BONES WHEN A PIECE OF METAL PENETRATED HIS GLOVES & LODGED IN HIS LEFT INDEX FINGER. HE WORKED WITHOUT RESTRICTIONS UNTIL 3/31/06 WHEN AN OPERATION WAS PERFORMED TO REMOVE THE FOREIGN BODY.
He was using a cut off bar to uncouple two motors. The coupler were aon a bind, when they came uncoupled he caught his left ring finger between the cut off bar and the ear on the motor motor causing a laceration requiring 5 sutures.
HE WAS USING A RATCHET TO INSTALL A MOTOR ON A AIR COMPRESSOR, THE RATCHET SLIPPED AND HE CAUGHT HIS RIGHT LITTLE FINGER BETWEEN THE RATCHET AND FRAME ON THE COMPRESSOR CAUSING A LACERATION REQUIRING FIVE SUTURES
EE WAS USING A KNIFE TO CUT BELT, KNIFE SLIPPED AND LACERATED LEFT THIGH REQUIRING FIVE STAPLES
He was using a screw driver to install a light plug, the screw driver slipped and struck his left thumb causing a laceration requiring five sutures.
He was using a pry bar to remove the pot out of a loading machine. The bar slipped and his left middle finger was caught between the bar and the frame of the loading machine causing a laceration requiring 4 sutures.
INJURED WAS CUTTING RUBBER BELT WHEN THE KNIFE SLIPPED LACERATING HIS LEFT HAND BETWEEN THE THUMB & FOREFINGER, REQUIRING 3 SUTURES.
HE WAS USING A BAR TO ADJUST THE FORKS ON A SCOOP AND FELT PAIN IN HIS BACK. HE EVENTUALLY WENT TO A DOCTOR AND WAS DIAGNOSED WITH MUSCLE STRAIN.
CUTTING A NYLON ROPE FROM THE SLOPE BELT, THE KNIFE SLIPPED CAUSING A LACERATION TO THE LEFT PALM REQUIRING 3 SUTURES.
PULLING RAIL SPIKES WITH A SPIKE BAR, BAR SLIPPED AND CAUGHT HIS RIGHT INDEX FINGER BETWEEN THE BAR AND RAIL CAUSING A FRACTURE OF THE RIGHT INDEX FINGER.
WHILE STANDING IN STAGE LOADER REACHING OVER CRUSHER TO TIGHTEN BOLTS, EE FELT A PAIN IN HIS RIGHT RIB CAGE.
He was changing bits on he shear, using pliers to hold the snap ring open to take out a drum insert. When he used a hammer to hit the bit, the snaping flew up and hit his chin.
A MECHANIC WAS USING A CHAIN HOIST TO PULL A FLOTATION CELL PUMP. HE HAD THE CHAIN HOLDING THE PUMP WHILE HE TOOK THE BOLTS OFF. THE CHAIN HOISTSLIPPED PINCHING HIS FINGER (RIGHT INDEX) CAUSING A LACERATION.
A MECHANIC WAS USING A RACHET TO TIGHTEN A BOLT ON A MAST JACK OF A ROOF BOLTER. THE RACHET SLIPPED AND HE STRUCK HIS RIGHT HAND AGANIST THE BASE OF THE JACK CAUSING A LACERATION.
A MOTORMAN WAS COUPLING A CAR TO A MOTOR. THE COUPLER WAS SLIGHTLY LOWER AND HE USED A "DISCONNECT BAT" TO RAISE THE COUPLER WHILE ANOTHER MOTORMAN COUPLED THE CAR. WHEN THE COUPLERS CONNECTED THEY WERE MISALIGNED AND THE BAR HE WAS HOLDING SWUNG INTO THE CAR CATCHING HIS THUMB BETWEEN THE SIDE OF THE CAR AND THE BAR.
WAS USING A BAR TO CLEAR A CHUTE THAT WAS PLUGGED WITH ROCKS. THE ROCKS CLEARED THE CHUTE AND HIS FINGER WAS CAUGHT BETWEEN THE BAR AND CHUTE.
WAS HOLDING THE HEAD OF A BOLT WITHH A COMBINATION WRENCH WHILE USING AN AIR POWERED IMPACT WRENCH TO TAKE NUT OFF BOLT. THE COMBINATION WRENCH SLIPPED FROM ITS POSITION AND HE CUT HIS LEFT RI NG FINGER ON THE BOLT.
ELECTRICIAN STATED THAT WHILE USING A RATCHET TO TIGHTEN A LUG HE FELT A PAIN IN GROIN. CONTINUED TO WORK UNTIL APRIL 24, 2003 WHEN HE WENT OFF WORK FOR HERNIA SURGERY.
