Showing 50 of 65Was working on putting a shuttle car conveyor chain back together. Co-worker was tightening come-a-long. The chain attached to the flight (not the come-a-long chain) broke loose striking employee in the hard hat.
Employee was pounding a wedge in on the side of a wall when the hammer missed the wedge and struck his left thumb. The employees left thumb was against a cement block when he struck it.
Employee received 6 sutures when cutting a band holding a lift of bolts. The band struck the employees left forearm causing a cut 3" long and 1/4" wide.
Employee was changing bits on longwall shearer using a bit tool when he struck his right hand with a 3 lb hammer causing his hand to be forced down on bit tool. A laceration and fracture to right pinky finger resulted from the force.
The employee was installing a connector into the end of his roof bolt drill steel. When tapping the end of the connector with a hammer, the connector flew out striking his upper lip, 4-sutures were required. The employee returned to work his next scheduled shift.

He continued to work at his regular duties & continued to have pain. He was removed from work on 7/14/11 for shoulder surgery. His MRI shows severe degenerative changes.
He was using a wire rope come-along letting the screen down when the handle of the come-along struck him on the lower lip causing a laceration requiring 5 sutures.

He was using a screw driver to install a light plug, the screw driver slipped and struck his left thumb causing a laceration requiring five sutures.

A MECHANIC WAS USING A RACHET TO TIGHTEN A BOLT ON A MAST JACK OF A ROOF BOLTER. THE RACHET SLIPPED AND HE STRUCK HIS RIGHT HAND AGANIST THE BASE OF THE JACK CAUSING A LACERATION.
A MOTORMAN WAS COUPLING A CAR TO A MOTOR. THE COUPLER WAS SLIGHTLY LOWER AND HE USED A "DISCONNECT BAT" TO RAISE THE COUPLER WHILE ANOTHER MOTORMAN COUPLED THE CAR. WHEN THE COUPLERS CONNECTED THEY WERE MISALIGNED AND THE BAR HE WAS HOLDING SWUNG INTO THE CAR CATCHING HIS THUMB BETWEEN THE SIDE OF THE CAR AND THE BAR.
WAS USING A BAR TO CLEAR A CHUTE THAT WAS PLUGGED WITH ROCKS. THE ROCKS CLEARED THE CHUTE AND HIS FINGER WAS CAUGHT BETWEEN THE BAR AND CHUTE.
WAS HOLDING THE HEAD OF A BOLT WITHH A COMBINATION WRENCH WHILE USING AN AIR POWERED IMPACT WRENCH TO TAKE NUT OFF BOLT. THE COMBINATION WRENCH SLIPPED FROM ITS POSITION AND HE CUT HIS LEFT RI NG FINGER ON THE BOLT.
ELECTRICIAN STATED THAT WHILE USING A RATCHET TO TIGHTEN A LUG HE FELT A PAIN IN GROIN. CONTINUED TO WORK UNTIL APRIL 24, 2003 WHEN HE WENT OFF WORK FOR HERNIA SURGERY.
