Employee was removing a bushing from a shaft and the bushing quit moving. He then used a screwdriver to pry on the bushing when it suddenly broke free and slid down the shaft and pinched his finger between the bushing and the manlift he was standing in causing cut to his right index finger. He was taken by private vehicle to the emergency room for evaluation and received 1 stich.

Employee received a laceration which required stitches to his right index finger, from the lid on a can of soup while attempting to open it with a pocket knife.

Employee was welding the floor plate on Komatsu truck box. While he was chipping slag off of a welded area with a chipper, he felt something go into his right eye underneath his glasses and face shield. Seen by MD and had a small piece of metal removed from eye.

Employee was tightening a head bolt on a drill using an allen wrench and hammer. Hammer bounced off of steel rack hitting employee's knuckle of his left index finger causing a laceration that required stitches.

Employee was attemting to repair a headlight problem on unit 3056. While in the process of cutting the boot off the back of the headlight, his pocket knife slipped and cut the side of his left thumb. Laceration required stitches.

While tightening nut on 789 left rear dual pilot stud, torque wrench pinched right hand between torque gun and inside of wheel rim. Seen in ER and placed on restricted duty for soft tissue injury to the palm of right hand.

EMPLOYEE WAS STRUCK IN THE ARM WITH A SLIVER OF METAL WHICH ORIGINATED FROM THE REMOVAL OF A BEARING RACE WHICH WAS BEING WORKED ON APPROX. 8' AWAY USING A DRIFT PUNCH. EE WAS SEEN BY MD AND THE SLIVER OF METAL WAS REMOVED FROM HIS FOREARM.

EE WAS USING A LIFTING CABLE WITH SHAKE OUT HOOKS TO REMOVE PLANKS FROM LOWBOY DECK. EE HOKED 1 CABLE TO A PLANK, OTHER 3 CABLES WERE WOOSE AND NOT SECURED. ONE LOOSE CABLE CAUGHT ON BEAM AND WHEN EE TIGHTENED UP HOOK IN PLANK, HOOK THAT CAUGHT ON BEAM CAME LOOSE AND STRUCK HIM IN THE FACE.

Employee has had a history of spinal sterosis w/degenerative disc disease of his neck & underwent a multi-level cervical fusion in 1999. EE has had ongoing problems with pain & numbness, both arms. EE started taking sick leave on 11/04/04. On 12/20/04 we were notified by the state the EE alleges time off was from an incident which occured on 10/04/04 from pulling on a wrench.

EMPLOYEE WAS USING A POCKET TYPE KNIFE TO CUT TIE-WRAPS LOOSE FROM A HEAVY EQUIPMENT FUEL LINE. THE KNIFE SLIPPED THROUGH TIE-WRAP & MADE CONTACT WITH EE'S LEFT ARM SEVERING HIS RADIAL ARTERY. EE HAD SURGERY TO REPAIR ARTERY & WAS RELEASED TO NORMAL DUTIES AS A MECHANIC LEADMAN, NO RESTRICTIONS.

EE WAS REMOVING A PIN FROM SHOVEL LADDER TO MAKEREPAIR TO THE LADDER. WHEN EE KNOCKED PIN OUT OFTHE PUNCH HE WAS USING, IT SHIFTED RESULTING IN LEFT FINGER BEING CAUGHT BETWEEN PUNCH AND LADDER FRAME.

MAG SEPARATOR PAN. THIS BECAME REPORTABLE ON AUGUST 8 WHEN EE SOUGHT MEDICAL TREATMENT FOR INFECTED LEFT ELBOW.
