The #1 conveyor belt chute at the Ceredo Dock plugged off with coal. Employee was using a sledge hammer to hit the side of the chute on the upriver side of the middle level of the transfer structure to knock loose the frozen coal when a piece of metal about 1/4x10x34 inches in size hit his left hip and back area, causing a bruise/contusion.

Kanawha River Terminals · Caught in, under or between a moving and a stationary object

Employee was attempting to adjust broom bristle on sampler tail piece with pry bar. Pry bar snagged moving belt and smashed employee's fingers between pry bar and belt resulting in fracture to left ring finger and laceration to left pinky finger requiring 4 stitches.

Employee was removing lodged lump of coal with slate bar. In an attempt to free the material, the slate bar & lump fell out of the bin area striking employee's right leg ultimately causing a scrape on the leg. Employee refused medical attention initially, but went to family doctor on 5/26/09 because of redness. Doctor put employee off work for rest of week for infection.

Employee was attempting to adjust spotlight on off-loader barge by tossing a rope around the light bracket. During the tossing motion, employee pulled muscles in his back. Employee didn't inform mgmt. that he missed work on 6/25, due to this incident, until 7/3/07.

EE was installing a new run of conduit. During the conduit run, a bracket from the old conduit needed to be removed. The anchor bolts from the old bracket were loosened & EE tried to pry the bracket the rest of the way out of the wall. At this time, the bracket fell and grazed EE's left index finger, resulting in a laceration requiring 3 stitches.

EE was tightening an anchor bolt on a counterweight guide beam. The crescent wrench he was using slipped off of the anchor bolt, causing EE to hit an adjacent anchor with his right fifth digit finger. This resulted in a bruise to the finger and EE was given 3 days off by his attending physician, therefore resulting in a lost time accident.

Employee was attempting to close the hopper door on a railcar with a pry bar. During this time the pry bar slipped and struck the employee in the forehead resulting in a laceration .
