Employee was flex lancing a vertical 6 inch pipe from the bottom. The opening was approximately 12 inches from the ground. As EE was flex lancing a piece of the product came lose falling and pulling the flex lance out. The tip of the lance struck EE's left leg below the knee causing a laceration. several times for follow up all ending in EE being released to light duty.

Employee was using an air chisel to descale a box in Precipitation. While doing so, the air chisel shifted and dropped, catching the employee's finger between the chisel and metal structure resulting in a fracture and laceration.

Employee was scaling hopper with pneumatic tool. When placing the tool on the ground, the employee felt a sharp pain in lower back. Employee was later referred to physical therapy.

While replacing a long cylinder on #9 press, the winch was attached to the rear of the long cylinder after the center mount had been secured to pull the chamber back so the pin could be installed. The torque on the winch was released and the long cylinder pivoted downward pinching EE's foot causing a laceration on his foot behind the steel toe of his boot.

The employee was removing scale from a valve with a rivet buster. When the scale broke free the rivet buster dropped pinching the employee finger between the rivet buster and the valve. The employee incurred a linear fracture in the distal phalanx.

Using overhead crane with cable employee placed the cable on one of the lower bolt & nut on press head. This was in an effort to help turn the press head for opening. Using the remote control in right hand for the crane, he inadvertently tightened the hoist cable jamming is left hand between cable and press head. Severely bruised left hand. He was placed on restrictive duty.

Workers were unbolting the check valve on the Dilution steam line to V1. They cut the second to last bolt and a little cold water came out of the line. They then cut the last bolt and that is when the line started blowing steam. One employee was struck by the steam.

EE was installing a 14" x 12' double 90 tail leg with a spacer. While trying to install the spacer, the spacer kept catching the gasket. EE reached in with his hand and at the same time the crane was lifted and squeezed the fingers on his left hand. Surgery was performed the same day. He returned to work with restrictions.

EE was descending the stairs exiting 102 gantry boom. He missed the second step with his right foot putting all of his weight on his left leg. EE stated he felt a pop behind his left knee and a sharp pain. He was transported to medical dept. Knee strain, restricted work duties.

EE was working on descaling 2 to 3 hopper with a #2 gun. Scale broke free from the wall hitting the bit that is used in the gun causing the gun to be pushed back. His index finger got caught between the gun and side wall. He did not report to medical until June 15, 2012. His finger was fractured. He returned to work with no restrictions on 6/15/12.

Hardy and a co-worker were picking up a welding machine. Hardy pulled the jack up and co-worker was holding the welding machine and dropped it. Hardy then picked up the welding machine to hook it to the scooter and felt pain in his back. Lower back strain. Restricted work duties.

Employee was grinding on a 14" pipe using a 4-1/2" angle grinder. He was repositioning the pipe when he lost his grip on the grinder. He reached for the falling grinder and jammed his hand into the work table causing a fracture to his right middle finger. He is restricted to "No use of right hand". These restrictions cannot be accommodated. He is now a Lost TIME case.

EE was using the drill press to drill holes in a bracket. He attempted to move the bracket in the vise and his glove got caught in the drill bit. His left index finger got wrapped in the glove and caused a severe laceration of the left index finger requiring 17 stitches. His duties are restricted.

EE was attempting to polish a shaft on a 3/4 horsepower motor with sandpaper when the shaft grabbed the sandpaper and his right glove twisting it off his hand. EE suffered a broken finger. He is not restricted and has not lost time from this event. These events took place on 3/25/11, but company was not made aware of this until 4/12/11

While attempting to balance hydraulic jacks to be loaded on pallet, the jack tipped and EE twisted his right thumb. On 3/15/11, he was ordered to wear a splint and his work is restricted to no use of right hand by his orthopaedist.

EE filled the S. Primary Feed with caustic when drain was open. When he approached the drain valve to close it, the ore he was standing in washed from under his feet causing him to slip into the caustic that had pooled. The caustic was deeper than his boots, contacting his feet, causing chemical burns. He was transported to the hospital burn unit in Baton Rouge, La.

EE was working on #3 Settler scaling out the launder, when the gun of the scaler became jammed. The pressure of the air from the gun caused it to jam the left middle finger between the gun handle and the wall. X-rays revealed fractured left middle finger.

Injury was blowing down the 650# pressure transmitter under #1 power boiler. The trench under the power boiler filled with hot water and was missing a piece of grating. He stepped into the trench and received 2nd degree burns on his left foot and ankle from the hot condensate. This injury required medical treatment and time away from work.

While working on welding repairs of #3 kiln Unax coolers, hot metal from area being welded fell in left shirt sleeve causing 2nd degree burns on his left upper arm. Cleaned, then applied Silverdine cream 1%, applied & dressed with adaptic & gauze.

Employee was cleaning out a ditch, too close to the edge and when he casted the machine slid into the ditch. He was taken to GA Medical where he was checked out. He then went to the doctor, he was diagnosed with a sprained right thumb.

On 10/29/08, EE worked through the day using a hand grinder on piping. On 10/30/08, EE reported that his shoulder was hurting and he believes this is related to the work on 10/29/08. EE was sent for medical evaluation and placed on restrictions.

Cherry Picker was making a lift to reinstall catwalk, rigging on catwalk slid and came unhooked. Section of platform came down and hit employee in the lower right back, causing a contusion.

Employee was hit in the right eye with a piece of metal while working on a break in job. Eyelid swollen upon leaving jobsite. Employee was kept awake from the pain. Reported to Medical the following morning and was sent to Williamson Eye Center. Diagnosed with Lid Edema, SPIC. Prescribed Zylet.

Employee (General Repairer) reported irritation to his eyes on the morning of August 21. He had peformed fitting work on the 20th, working near a welder. Due to the degree of irritation, he was sent to an ophthamologist for an exam, was provided prescription eye drops, and restricted from welding.

Operator was removing slag from inside tank wall with (Grant BDRE) when a piece of slag dislodged from above hydraulic hammer, the slag traveled down length of boom and struck the operator in the left shoulder, fracturing his left scapula.

Employee was welding the bottom boot of AG-6. Upon returning to work the next day, he stated that it felt like a flash burn in his left eye. Dr.diagnosed foreign body in left eye. It was removed, a bandage contact lens was applied and Polydex drops were prescribed.

Employee was cleaning panfeeder on 102 gantry. Was using scraper to clean scale off the walls of panfeeder hopper. Was jabbing at scale and struck right hand, right side of his hand and little finger & joint on angle iron cover for indicator. Diagnosed with fracture to knuckle. Surgery scheduled for 10/10/05 to repair.

Employee was grinding V2 Digester outlet block valve and was preparing to remove locking pin from valve when his supervisor opreated the eliminator to release locking pin. The anchor for the eliminator hit him on his lower lip. He was sent to first aid & the company doctor. He received stitches to close the laceration & returned to work with no restrictions.

Employee was fabricating parts in the lathe. He was preparing to take the sharp edge off of a piece of material and then mic it. As he put the sandpaper to the material, it caught, pulling his right index finger into the spinning material. Medical treatment (stitches) were required to close laceration.

Employee was realigning west pump off unit. He forgot one valve open to caustic mix tank and put tank 31 pumping. Caustic mix tank ran over, spilling caustic into middle unit. To remove spilled material, employee tried to back flush sump pump and caustic got in his boot causing burning to right foot, below ankle. Sent to St. James Hospital, treated, released.

Employee was tightening snubber block at end of 101 Gantry boom. Using air impact gun to tighten 6 bolts of the snubber block on the close line. When attempting to pull slack on air line, felt sharp pain in his back. Went to medical for treatment, & continued to experience back pain. Determined restrictive duty on 2/10/05.

Employee was grinding pipe patches. As he completed his work, he layed the grinder on the table. The grinder was still turning and caught in his left glove. It pulled itself into the left index finger, cutting the top of his finger. Medical treatment was provided.
