Showing all 11Jetter hose had a weak area that was not recognized in initial shift inspection. Upon use, the hose burst injuring operator on the left hand. Was treated at local medical center for hand injuries and referred to secondary care at burn center in Augusta. Care was advised to apply ointments and wrap. Return for follow-up in two weeks.
Employee unsure of when incident happened. Stated EE woke up on the 7th with eye irr/swelling. Employee was working on roof during windy conditions with grinding being performed by others. Employee was wearing safety glasses while working. Optometrist removed small metal fragment from employees eye and prescribed antibiotic eye drops. No restrictions were given by physician.
Teammate attempting to cut board on a benchtop in the maintenance shop. Using angle grinder, teammate worked on a section which was not properly secured. Unsecured section shifted caused mechanic to lose control of board. Holding the grinder in EE's right hand, reached out left hand accidentally made contact with the rotating wheel, resulting in lacerations to two fingers.
Employee was cutting a bolt(S) off chassis, was wearing safety glasses, but got some debris in eye.
Maintenance employee was holding a piece of flat bar being cut by coworker when employee felt something pass behind the safety glasses and into eye. Employee when to local ER and had a small piece of metal removed that was imbedded into the left eye.
Pipe being removed was covered in insulation. Corrosion of pipe was more than thought due to not being visible. Weight of upper end of pipe was heavier than expected due to kadin build up. When pipe was lifted with crane it broke in to 3' below the crane rigging which transferred the majority of weight to the upper side which was being stabilized by a rope as a safety measure.
While operator was performing on a smoothing bucket unit, the bucket struck a buried metal pipe on the ramp which caused the unit to stall which forced the operator forward causing his left cheek to strike a mounted CB radio.
Foreman was cleaning up the cut with a Cat D6M dozer. He went over a small hump of dirt, his cell phone engaged the parking brake on the left arm rest causing the dozer the stop suddenly. The foreman had his seatbelt on, the stop caused his neck to snap forward. He was stunned for a few minutes, then drove himself to the doctor.
Worker received a piece of foreign matter in right eye while cutting a piece of metal with demolition saw. Worker had safety glasses and face shield on at time of accident. Demolition saw had all guards in place.
Employee was helping operator set up the drill rig to begin drilling. As the operator was lowering the kelly into the turntable the employee allowed his right index finger to be pinched between the kelly block and the turntable. Weather conditions were sunny, dry, and warm.
