Driller was lowering core tube retraction tool (also known as the overshot tool) down into the cased hole. As the driller was returning the overshot tool to its resting location, EE inadvertently tripped hydraulic lever to the "up" position, rather than the "down" position, which pulled the driller's arm upwards.
The employee was fabricating a drill steel carrier onto a Skyjack fork lift. They had been using a die grinder to oblong some holes when they felt a prickly sensation in glove on right thumb. They found a metal sliver in the pad of their thumb that they removed. The next night felt discomfort in right thumb and was taken to a medic.
EE was working on new basket installation on KBT20, was welding, cutting and grinding using all PPE. EE did not notice irritation to L eye until later in evening. EE was not sure if eye was scratched or irritated. EE sent to town clinic on 7/19. 7/26 received a definitive diagnosis from Dr. that the debris was imbedded and removed easily w/instrument.
Drill helper was hand threading drill rod onto drill string. Driller moved rotation unit down before drill rod was threaded and helper was clear. Rotation unit caught drill rod forcing it down pinching drill helpers foot between drill rod and drill deck.
Employees glove got caught on a rotating drill steel and broke his right forearm.
The employee was using a 9" grinder cleaning up a metal box and contacted his right hand with the grinding disk.
Miner was using an air impact gun to remove a broken bolt from the Z arm cap on an underground 9yd loader.
While clearing a pipe by the Pregnant Solution Tank with a 2" air line the Miner lost control of the air line and the fitting slammed into the Miner's right leg causing a hematoma.
Two miners were bolting a heading and a third arrived to help. The miners were discussing a plan of work when a jackleg drill fell over striking him on the left leg causing a fracture in the left ankle.
EE was welding floor, a worker welding out of buggy was pulling up a 9" grinder by the cord when it seperated from the extension cord falling and striking EE in the shoulder blade.
Employee was operating a lathe completing what appeared to be the final stage of machining 3/4" round steel rod. Employee was using a piece of emery cloth polishing the steel rod, it appears that the emery cloth either caught on the steel or wrapped over its self and pulled employee's left hand into the lathe while it was running
Employee had completed his job task of pressure washing off equipment within the wash bay. Employee then attemped to clean off his gloves using this pressure washer (approx. 3000 PSI). Water penetrated his left glove and injected some of the water through a laceration to his middle finger.
