In verifying operation of air actuated valve, employee placed hand on or near the valve seat and valve closed, pinching the middle finger of the right hand. Doctor diagnosed an open wound of right middle finger with damage to nail. Treatment included subcutaneous tissue repaired with 2 sutures and an injection of Ketorolac. Employee discharged as fit for duty without restrictions.

Showing all 3Miner was cutting off electrical conduit down from the batch plant, unistrut clamp was not secure and the conduit fell about 4 to 5 feet hitting employee on the hard hat. Upon impact, the hard hat suspension liner pressed into miners forehead, causing laceration. Miner received sutures on open wound.
He was using the cutting torch and some of the dirt flew under my safety glasses and into his eye. Dirt in Right eye.
