An employee was using a 4-inch angle grinder when debris went underneath EE's safety glasses and irritated EE's right eye. The employee stated EE could not use face shield at the time because EE was in very tight quarters while grinding. The employee was taken to E.R., where EE's eye was flushed and the debris removed. EE was given a full release with no restrictions.

Employee using 4.5" grinder had small piece of debris go into right eye. The employee was grinding on a tail shaft when a small piece of debris went under employee's safety glasses and entered the right eye. Employee stopped grinding and reported incident to supervision. Employee was taken to E.R. to assess eye injury.

Employee was cutting a bolt on some piping with a grinder and the grinder got caught in the bolt which caused it to jerk when ee reacted. The employee ended up with three lacerations on the inner part of knee.
