Two Drill operators were changing the sub on a drill. At approximately 16:15, the second operator positioned the J wrench on the sub to break it loose from the drill steel. The drill operator inside Cab went through neutral on the controls and engaged reverse rotation of the drill steel. As a result, the J wrench swung back and pushed the operator's foot beneath the carousel.

Showing all 2Technician was using an impact driver to remove a pin from the articulation cylinder. The pin suddenly came loose, crushing the technician's left index finger between the back of the driver and the engine compartment door. Left index finger had been previously amputated to the first knuckle. The crush caused an open fracture to the second finger bone.
