Employee was drilling 1-5/8" holes and when in the process of changing out the drill bits, the head released and pinned EE's Right-Hand little fingertip between the Jacobs chuck and the plate EE was drilling. This caused a partial amputation of the Right-Hand little fingertip.

Hydraulic ram was placed on a lug mounted to the side of the dragline. The ram slid off the lug. The safety line attached to the ram caused it to swing and contact EE's left hand.

While brush cutting with tractor and 15 foot brush cutter operator hit an unseen rock which was propelled from the cutter through the back glass of tractor striking the operator on the right side of head causing a laceration. Operator was transported to clinic where laceration was stapled shut.

A contractor employee dropped a 1" impact approximately 2' to 3' and struck a Luminant employee on the lower left side of EE's back, just under the bottom rib. Physician performed x-ray with negative results. Employee was released to work with restrictions.

Employee was air gouging to remove the wear plate from a D9 dozer's blade. As employee was making the last cut, the stored energy released the top of the plate and it struck the employee on the right shoulder causing them to lose balance and fall/ Hard hat came off and head struck the new wear plate which was staged nearby. Employee has 5 years mining experience.

EE was drilling holes in a dragline swing case seal so it could be removed. In the process,felt something in left eye and tried to wash it out. EE was transported to the ER. Next day EE was seen by a eye specialist to remove a small piece of metal from the eye, a prescription was given for pain/eye drops and EE was instructed not to return to work for at least 24 hrs.

Fans were turned on inside the D-35 dragline machinery house after a 4 day outage. The fans caused dirt/debris to enter the right eye of a mechanic who was working near the #1 swing. EE did have safety glasses on. EE didn't report the event until 11/6/2017 as eye continued to worsen. EE received prescription eye drops from the injury.

Mechanic was trying to cut a 3" rod bolt off that was stuck. Was using a portable saw and had made two cuts. The saw blade caught and kicked the saw back cutting thru the glove cut left ring finger at the first joint. 3 stitches were applied. No restrictions. All PPE was properly being used-Face Shield, Gloves and hearing protection. Saw had all safety devices in place.

Carrier retainer bolts on 8750 dragline were being removed with a 1" air impact and a 2" socket. 2 or 3 bolts had been removed and on the next bolt the socket broke into 2 pieces. Part of the socket contacted left wrist, EE didn't release the impact and it torqued EE's right elbow. Restrictions were issued by Dr. and this became recordable and lost time.
