While loading 14-foot jumbo steels onto the rack, the jumbo began reversing; a steel caught on rock, sprang free, and struck the IP's left knee.

Showing all 12The IP was operating mucker at a working face when the mucker bucket made contact with the rib. This contact caused a jolt and lower back pain. The IP felt it unremarkable, completed shift, went to bed and woke up in pain before the next shift. The IP was seen at our onsite clinic. After 3 days sick in quarters, the IP followed up at home with their Dr. Where employee remains.
IP was cutting anchor (deadman) for drill rig with cutting wheel. While spooling down preparing for next cut, the wheel contacted their knuckle resulting in small laceration to knuckle. Shifter notified immediately, walking boss took IP to medic. Medic treated and used 2 stitches. Gloves, face shield and guard were in use at time of incident.
Dust/debris fell into eye behind glasses while installing cable bolts. IP thought IP had removed debris, but reported to clinic at 5:45 PM, IP went to a specialist the next day for further evaluation, and removal of debris.
Employee was communicating with co-worker who was running the porta power to break bead off tire. The employee told the co-worker to apply pressure and it began pushing the bead off the rim. The tool began to fall and the employee grabbed the tool to catch it. The jack then suddenly blew off the rim and pinched the employee's hand between the stud and the jack.
The IP was holding a piece of metal stock with one hand while attempting to use the other hand to operate a hand held cutting tool. The cutting tool kicked back during the cutting of metal and contacted the IP finger causing injury requiring sutures.
Helper was in the task of trying to remove the core barrel out of the head. Barrel was coming out of the head when the balance of the barrel shifted causing it to angle toward the driller and control panel. The helper attempted to divert the barrel and in the process the barrel hit the control panel and made contact with the helpers right wrist and hand.
THE CORE HOLE WAS TD'D. DRILLER AND HELPER WERE PULLING CORE BARRELL OUT. CORE BARRELL GOT HUNG UP AND HELPER ASKED DRILLER TO BRING THE CHUCK DOWN TO HELP EASE THE AWKWARDNESS. AS THE DRILLER FED THE CHUCK DOWN THE CARRIAGE PLATE CAME INTO CONTACT WITH HELPER'S FOOT THAT WAS HANGING OVER THE DECKING CAUSING MINOR FRACTURE AND LACERATION TO BIG AND INDEX TOE ON LEFT FOOT.
IP grouting cable bolts when hose came loose from tube spraying grout into IP'S face. IP did not report incident to operator until Aug 3, 2021.
Welder struck in face with line boring hydraulic drive motor couplings.
Employee was operating an overhead crane while assisting with the installation of a loader cutting edge. The strap broke, the cutting edge fell, and bounced off EE's left steel toe boot. This took place inside the surface shop on a concrete floor. The employee was seen in the emergency room the following day and diagnosed with a broken left big toe.
On 5-7-2020 associate was mixing soup using an immersion blender, the associate reached for seasoning to add to the soup, and lost control of the mixer, the mixer was failing out of the soup kettle when the associate tried to catch it with EE's hand which in the process came into contact with the spinning blade of the mixer causing multiple lacerations.
