A rejected bag hung up at the top of the rejects chute in Robo 2. The operator used a ladder to reach into the reject chute to free the bag but when EE reached for the bag the cutting wheel started. The operators right thumb contacted the cutting wheel resulting in a cut.

Employee was in Haul Truck #28 being loaded by 559 Shovel on the Phase 6 6000 Bench. While being loaded a large boulder rolled off the bucket of 559 shovel, the boulder landed in the bed of the truck. The impact jarred the truck operator.

A Shipping Center operator was packaging material in paper bags when a 50 lb. paper bag was hung up on the bag chair still connected to the fill spout. The bag began overfilling and the operator attempted to move the bag at which point the bag expanded beyond capacity and erupted. The operator was wearing safety glasses; however, material entered the operator's right eye.
