Haul truck was struck by a shovel dipper door jostling the truck driver. The truck driver had pre-existing degenerative disc problems which were aggravated by this incident. She sought chiropractic treatment.

Employee was lifting a jackhammer out of a crusher and he had his left hand on the bit. The trigger was inadvertently contacted while lifting it out, and the jackhammer was activated momentarily. The bit cut through his leather glove and cut the palm of his left hand about 3/4", and sutures were required to close the laceration.

HE WAS OPERATING THE RIGHT DRILL HEAD ON THE 001 SECTION DRILL. HE HAD DRILLED THE HOLE AND WAS INSTALLING A CABLE BOLT. HE HAD PUSHED THE BOLT INTO THE HOLE AND WAS ATTEMPTING TO PLACE THE BOLT HEAD INTO THE TIGHTENING WRENCH WHEN HE CLOSED THE STEEL GUIDE ON HIS THUMB.

Employee complained of back pain at the end of his shift, although there was no single event that caused any pain. A doctor examined him and concluded that a multitude of factors have contributed to this condition over time including weight, physical condition and operating heavy equipment. He was placed on bed rest and prescribed physical therapy.

HAND, THE TORQUE WRENCH TRIGGER INADVERTENTLY STRUCK THE SEGMENT CAUSING THE WRENCH TO TURN ON. FINGERS WERE PINCHED BETWEEN THE TORQUE WRENCH BRACE & THE MACHINE SPROKET.

WHILE ADJUSTING THE BELT TENSION ON A MOTOR RUNNING A BELT DRIVEN MACHINE BORING HEAD, THE MOTOR WAS TURNED ON BY THE OTHER EE WITHOUT CHECKING TO SEE IF THE INJURED EE HAD HIS HANDS AWAY FROM THE BELT. WHEN THE MOTOR TURNED ON IT GRABBED THE EE'S HEAVY WELDING GLOVES AND PINCHED IT BETWEEN THE BELT AND THE SHEAVE. RESULTING IN A SMALL FRACTURE TO THE LEFT INDEX FINGER.

He was installing roof bolt into previously drilled hole. As he pushed bolt into hole, it caught on lip of hole & bent the bolt. He struck bolt with tightening wrench, in hopes that bolt would jump into the hole, but instead, the bolt came loose & struck him on lt wrist. He was instructed to follow up w/his dr on next day & returned to work on his next scheduled shift.

Employee was taking down a welding hooch after welding and grinding operations. He went home at the end of shift and woke the next day with his left eye hurting. He visited the hospital where a metal particle and rust ring were ground from his left eye. He was also given antibiotic drops for his eye.

After shift, employee discovered metal particle in his right eye. He had been using an air impact wrench while wearing safety glasses with side shields. He did not feel particle enter the eye. He reported to work next day, 7th. An attempt was made to wash particle out without success and he was sent to an eye specialist.

He was drilling a roof bolt hole when a piece of rock approx 12" by 15" by 1" thick fell from the roof and struck the drill head and broke into pieces. A small piece of rock bounced up from the drill head and struck him under his nose.

Building rock core for valley fill. Dozer pushing rock to excavator that had catch bench in front. Rock rolled from dozer. Excavator caught rock but rock sheared and half struck excavator. Rock broke windshied and glass struck operator's right shin causing laceration that required nine sutures to close. Employee returned to work next available shift.

During Part 50 Audit MSHA has determined that this injury should have been reported as the employee missed no days but worked two short shifts. Employee driving rock truck and being loaded by shovel was jolted forward in drivers seat when large rock struck the bed. Employee complained of neck pain but did return on his next work shift in his regular job.

HE STARTED MISSING WORK ON 10-07-03 AND WAS DIAGNOSED AS HAVING A FRACTURE TO 1 RIB. THIS OCCURRED ON THE 002-0 SECTION, 2 SOUTH PANEL AT THE #24 CROSSCUT.

WIND MAY HAVE CHANGED DIRECTIONS TO WHERE HE GOT A GOOD WHIFF OF SMOKE. HE FELT LIGHT HEADED AND SOME DISCOMFORT BUT RETURNED TO WORK. FOLLOWING DAY HE CALLED IN SICK BECAUSE OF SMOKE INHALATION.

HE HAD JUST DRILLED A NINE FOOT HOLE IN THE ROOFUSING 3 SECTIONS OF DRILL STEEL. AS HE WAS RETREATING THE STEEL OUT OF THE HOLE, THE TOP SECTIONOF STEEL GOT HUNG IN THE HOLE. HE HAD HIS HAND ON THE BOTTOM SECTION OF STEEL TO GUIDE IT AS HEUSED THE FEED TO RECONNECT THE STEEL, AND THE TOP SECTION FELL FROM THE HOLE STRIKING HIS FINGER.

THE EE FELT IRRITAT ION IN THE EYE BUT FIRST THOUGHT HE MAY HAVE SUFFERED SOME WELDING FLASH BURN. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 2 DAYS LATER WHEN THE EYE BECAME MORE IRRITATED & METAL DISCOVERED.
