ROOF BOLTER WAS BOLTING HIGH TOP WHERE ROOF WAS SHOT OUT FOR BEST HEAD WHNE CLEANING UP ROCK SOME OF BOTTOM HAD BUSTED UP WHEN BOLT MACHING OPERATOR STARTED TO BACK MACHINE UP HE LEFT HIS FOOT HANGING OUTSIDE DRIVERS DECK MACHINE WENT UP OVER ROCK AND CAUGHT HIS FOOT BETWEEN BOTTOM OF DECK & PIECE OF ROCK
