EMPLOYEE WAS HELPING LOADING EQUIPMENT( A VULCANIZING PRESS) ONTO OUR TRUCK. THE PRESS WAS 3'OFF THE GROUND WHEN THE EMPLOYEE WAS GUIDING THE PRESS AWAY FROM THE RAILING TO PREVENT FROM HITTING WHEN THE STRAP BROKE .THE PRESS CAME DOWN THE Employer's LEGS THEN LANDING ON BOTH ANKLES AND FEET.

Employee was installing a roof bolt when the bolt kicked out and struck his arm.
Employee inadvertently hit a paint can with his drill steel causing it to burst and cut his arm.
Employee was swinging the boom of roof bolter out to put up a rope bolt. As he was swinging the boom, his left hand was on the canopy and as a result his fingers were mashed between the canopy and the top.
Employee was attempting to tie beams to the top when a jack slipped out causing the beam to fall. As the beam fell, it struck him and caused him to fall back into the scoop bucket injuring his back.
On January 18th, 2010, the accident victim reported to his supervisor that on January 16th, 2010, he had become injured when he was backing up his dozer and it struck the ground forcefully, injuring his neck and upper back area (between his shoulder blades). The injured employee then began missing work as a result on January 19th, 2010.
Employee felt pain in his right shoulder when he was installing the roof bolt into the drill chuck.

The pin on the truck end of the duster worked out, allowing the duster to set down on the wheels. The duster was being jacked up to see what needed to be done to correct the problem when the jack creeled over and the duster struck the employee on his left foot resulting in a fracture.
Employee was hanging cable in the #4 entry on right side of bolter, with back turned away from bolter. Coworker was backing bolter out of the #4 entry and watching the cable on the left side of the bolter. Coworker never saw employee and backed over his foot and lower leg.
Excavator rolled off work bench when out slope failed. Employee suffered broken ribs and lacerations.
Employee was installing roof bolt in hole and had push part of the bolt up into the hole when he dropped head of bolter down to finish pushing bolt up, the bolt fell back out of the hole striking employee on his left index finger, resulting in a fracture to the end of his finger.

Drill steel became lodged in top, employee attempted to put wrench on drill steel and slipped hitting up lever catching index and middle fingers on right hand between wrench and drill steel. (required 11 stitches)
INJURED EE TOLD THE WITNESS THAT HE WAS GOING TO MOVE THE CABLE CROSSER. HE DID NOT HEAR HIM AND ACCIDENTALLY RAN OVER HIS RIGHT HAND WITH A ROOF BOLTER WHEEL.
EMPLOYEE REPORTED SEVERE BACK PAIN WHILE OPERATING A BULLDOZER.
Employee was split tramming single head roofbolter when he slew the machine around it bumped him on the left hip.
