Employee was diagnosed with a fractured wrist from an incident that occurred on 11/11. EE refused medical treatment at the time of incident. The employee was removing a lower roller when a bolt snapped, during loosening, causing the roller to push the hydraulic wrench and employees hand backwards. Employee continued to work until asking to seek medical attention due to swelling.

EE was dressing up an old road area by pushing dirt into a 3ft. washout when the blade aired out tipping toward the right front corner of blade causing the dozer to stop. Upon the verification that EE was wearing seatbelt correctly and because of EE's tall height and thin build allowed upper body to lunge forward causing head to hit the window latch.
EE was winching a generator into place (about 1 ft.) up to the Landoll Dropdeck Trailer to be loaded onto the trailer by an excavator when the chain connected to the generator in which the winch cable was hooked to broke.
Employee was operating pipe fusing machine. EE placed EE's hand on the hydraulic clamp to readjust. At the same time, using EE's other hand, EE operated the controls to close the clamp. The inside of EE's thumb was pinched between the pipe clamp and clamp support.
While grinding a weld off the bed of the dump truck utilizing leather gloves, repositioned ee to a squatting position while grinding in left hand requiring constant hand pressure, lost balance and caught himself with right hand on the deck of the dump truck. The grinder struck the floor and moved striking the top of right hand
Employee was using a hyd. jack with a 100lbs. metal spacer on top. When jack pressure was released the spacer fell hitting employees hand.
Employee was pressure washing a tractor. The tip of the pressure washer became clogged, and the employee swiped his hand over the tip of the wand and the pressure that was built up in the hose released and discharged water cutting his finger.
EE was grinding overhead and the grinding wheel came/broke off and cut through face shield, knocked eye safety glasses out of alignment and hit left eye
Two pumpers were connecting 6" pipe. The track hoe was connected to the opposite end of the connection. After communicating his intentions, the other pumper began to align the gasket. As he aligned the gasket, the track hoe operator was startled by material movement behind him. This made him hit the machine's swing, causing the two pipes to touch, pinching the employee's wrist.
He was using 6" boring equipment on the right outboard propel when the event happened. With the equipment on EE misplaced his hand causing it to become wedged between the smooth tube mount and the smooth drive drum. The distance between the tube and the drum is approx. 5/8". The employee was transported to the ER where he received 7 sutures to his right hand.
Employee was replacing cutting edges on a D6H Dozer blade. He was using a 3/4 inch impact with his right hand and his left hand was on top of the blade stabilizing himself. One of the plow bolts spun out and cut his left thumb - through his mechanic impact glove.
The operator was unloading the Cat 324D track hoe from the low boy trailer. When the operator got the machine to the back of the trailer he lifted the bucket off the ground and the track hoe tipped over on the boom side of the machine. The low boy was parked on uneven ground.
Employee was grinding in a small compartment when a flying particle entered his right eye.
Crew decided to place a rope spool on its side. As crane operator lifted, per signal person, top of load began to slighly lean towards the crane. Load was steadied by tagline. As he attempted to exit area, the bottom of load slid towards outrigger, hitting him in back of leg. The motion pushed his leg against outrigger, causing it to be pinched between the two.
Installing a carrier assembly in the right rear final drive. After I got the carrier assembly on the final I used the boom to hold the carrier assembly in place as I worked the carrier assembly inward. After the carrier assembly was bolted up I went to retract the boom and hit the work switch on the boom control. It swung the end of the boom into my face.
Cold temperatures, excess amount of clothing; standing close to saw horse to make fine cut. Saw kicked back cutting thigh. Because of excessive clothing guard got caught in clothing not being able to release
After grinding, the employee wiped his eye with his sleeved forearm. A small metal particle was found by the eye doctor in his eye.
Crew changing-out hoist ropes on 2570 W. Rope slipped free...swung forward and hit EE in the chest... throwing him backwards into the bucket pin. Rope hit EE from shoulder to hip and an abrasion in mid back.
An air motor on a lube pump tube had malfunctioned. The mechanic sprayed oil on the pistons to free them up. When he reattached the air line, the pump started running. His thumb was caught between the piston and the motor housing.
Operator of machine working in rough terrain when machine broke over a hump of dirt causing sudden jarring drop causing operator to receive upper back strain.
Employee was cutting trees 2" in diamiter. This particular tree was being cut 6" above his head. The weight & momentum if the saw caused him to swing down letting the saw come in contact with his leg. Causing him to hit his leg with saw.
EE WAS TRAMMING MINER BOOM FIRST INTO A 60 DEGREE CROSSCUT. HE STOPPED THE MINER AND BEGAN TO WALK PAST THE CUTTER DRUM AND SUDDENLY BECAME PINNED BETWEEN THE COAL RIB & THE CUTTER DRUM. HE ST ATED HE DOES NOT KNOW HOW THIS HAPPENED. MAINTENANCE DEPT CHECKED TRAMMING DEVICES ON MINER BUT FOUND NOTHING ABNORMAL. ACTUAL CAUSE IS STILL UNDER INVESTIGATION.
A PIECE OF DRAW ROCK FELL STRIKING HIM ON THE CALF OF THE RIGHT LEG, TWISTING HIS KNEE.
HE BACKED OVER ROCK OR BERM; DOZER JARRED HIM CAUSING HIS BACK TO HURT. HE SAT FOR A FEW MINUTES AND SAID HE WAS O.K. HE DID NOT WANT TO FILL OUT AN ACCIDENT REPORT. THE NEXT DAY HE WAS OFF FO R A DOCTOR'S APPOINTMENT BUT DID NOT SAY IT WAS FOR HIS BACK. STARTED LOSING TIME FROM WORK AUG. 11, 2003.
THE EE WAS USING A 4 1/2" PORTABLE GRINDER DURING THE DAY. HE WAS WORKING IN THE CHUTE @ 2A TO 4 CONVEYOR ALSO @ THE TAIL OF #4 BELT. SOMETIME DURING THESE OPERATIONS, 2 METAL PARTICLES GOT IN TO HIS LEFT EYE. HE DID NOT NOTICE ANY IRRITATION UNTIL AFTER SHIFT WHEN HE WAS WASHING HIS FACE. THIS COULD HAVE CAUSED THE METAL TO BECOME STUCK IN HIS EYE. HE WAS WEARING CO. ISSUE SAFETY
EE WAS GRINDING WITH A 4 1/2" GRINDER. THE EE STATES THAT HE IS UNSURE WHAT HAPPEN. ON FURTHER INVESTIGATION THE GRINDER WAS FOUND TO HAVE A WIRE WHEEL BEING USED TO CLEAN BOLTS. IT APPEARS AS THOUGH THE GRINDER KICKED-BACK. EITHER THE EE ATTEMPTED TO CATCH THE GRINDER WITH HIS FREE HAND OR THE GRINDER KICKED BACKINTO HIS HAND. HE ALSO STATED THAT IT HAPPENED SO FAST HE WAS UNSURE.
WHILE INSTALLING ROOF BOLT, A PIECE OF ROCK FELL. THE END OF THE ROCK STRUCK THE VICTIM ON HIS RIGHT FOOT.
OPERATOR OF THE ROOF BOLTER WAS BOLTING IN #6 RIGHT BREAK AND HE SWUNG THE CANOPY OUT AND STRUCK HIS JAW.
HE WAS OPERATING A ROOF BOLTER AND SWUNG THE BOOM AND IT HIT HIS RIGHT KNEE. HE BAGAN MISSING WORK ON THE 01/14/2003.
HE WAS SPINNING DRILL STEEL OUT OF THE DRILL HOLE, WHEN IT STRUCK HIM ON THE FACE.
EE WAS PREPARING TO SET A LARGE CAPACITY ROOF JACK, AS HE LIFTED THE JACK, IT SLIPPED FROM HIS GRASP AND PINCHED HIS RIGHT RING FINGER BETWEEN THE JACK AND A CRIB BLOCK.
HE GOT HIS THUMB CAUGHT BETWEEN THE DRILL STEEL AND THE ROTATION MOUNT.
HE WAS WALKING BESIDE ROOF BOLTER, WHEN THE OPERATOR SEUNG THE ROOF BOLTER AROUND. THIS CAUGHT HIM BETWEEN THE RIB & ROOFBOLTER.
HE WAS PULLING ON TRUCK AUGER, TO TURN IT OVER AND STRAINED HIS BACK. HE DID NOT START LOSING TIME FROM WORK UNTIL AUGUST 23, 2002.
INDIVIDUAL WAS GRINDING A PIECE OF METAL ON THE TAILGATE OF HIS TRUCK, WHEN A FIRE STARTED IN THE BED OF THE TRUCK. HE ATTEMPTED TO REMOVE A CONTAINED CONTAINING GASOLINE. THE CONTAINED RUPTUR ED LETTING GASOLINE ON HIS HAND.
ATTEMPTING TO STRAIGHTEN A 6'RESIN BOLT, AS HE PUSHED, THE BOLT ROTATED, CAUSING HIM TO PITCH FORWARD, HIS LEFT ARM CAME IN CONTACT WITH BOOM LEVER, THE BOOM RAISED, PINCHING FINGER BETWEEN BO LT & DRILL HEAD, COMPLIANCE NOT A FACTOR.
EE WAS INSTALLING ROOF BOLT & THE SPIDER PAN SLID DOWN THE BOLT, WHICH RESULTED IN A LACERATION TO HIS FOREARM.
WHILE TRIMMING A PIECE OF METAL WITH A TORCH THE METAL POPPED LOOSE & FELL WITH THE CORNER COMING IN CONTACT WITH HIS LEG.
HE WAS PUTTING THE DRILL STEEL AGAINST THE MINE ROOF WHEN A PIECE OF ROCK STRIKING HIM ON THE FOREARM.
EE WAS CUTTING IN TO RIGHT BREAK. A PIECE OF ROCK FELL FROM BOLT TO THE RIB STRIKING HIM ON THE LEG & ANKLE.
DRILL STEEL WAS STUCK IN HOLE, WHEN IT CAME OUT OF HOLE STEEL HIT FINGER AGAINST MACHINE.
WHILE INSERTING A ROOF BOLT, FINGER WAS PINCHED BETWEEN BOLT HEAD & BOLTER CHUCK. COMPLIANCE NOT A FACTOR, INATTENTION TO ACTIVITY A FACTOR.
INJURED WAS USING OVERHEAD CRANE. HE RAISED THE FRONT END OF MINER FRAME TO REMOVE CRIB BLOCKS, THEN LOWERED THE FRAME. AS HE BEGAN TO MOVE THE CRANE CHAIN HOOK CAUGHT FRAME, CRANE STALLED CHAIN HOOK SLIPPED FREE AND STRUCK HIM IN THE RT. SIDE OF FACE.
