WHILE BOLTING TOP, THE OPERATOR BACKED INTO A ROOF BOLT PLATE THAT WAS HANGING DOWN FROM THE TOP. THE SHARP EDGE OF THE ROOF BOLT PLATE STRUCK THE OPERATOR ON THE BACK OF THE NECK CAUSING A LA CERATION TO THE BACK OF HIS NECK.
