Cutting tooth wedges on drag line bucket w/ torch hot slag fell onto shoe burning hole into shoe and burn to top of left foot. Accident occurred on 6/19/12 did not seek medical attention until 7/2/12 off work starting 7/3/12
Dropping belly pan on D-11 Dozer-Pan struck left hand causing fracture to let ring finger.
Cutting pipe w/torch overhead, dropped torch and burning tip entered glove, causing burn.
