He was bolting 7-8 crosscut. He let the head down and said the joy stick stuck causing the head to come down on his foot and ankle. It caused a sprained ankle.

Employee was hit in face with bolter wrench. Employee had just finished put in roof bolt. Had wrench in hand left foot jack down. Canopy hit wrench, struck employee in face.

Bolter operator was on his knees operating roof bolter when a rock fell from the top & hit his back & then landed on his leg.
Track bolter helper was helping bolter operator pull a stuck drill steel out of a drilled hole. A piece of bad top came down landing on helper's back forcing him down to the ground.
WHILE ROOF BOLTING, A PIECE OF ROCK FELL FROM A CLAY VEIN AND HIT OPERATOR ON HIS BACK.
While operating roof bolter the operator raised foot jack which allowed the front of the machine drop down. The operator had his foot under that part of the roof bolt machine.

BOLTER OPERATOR WAS ROOF BOLTING IN THE 6 RIGHT X-CUT ON 1 EAST SECTION. HE POSITIONED LEFT HANDON TOP OF ROOF PLATE TO STEADY IT AND CATCH OFF-SET IN TOP. OPERATOR DID NOT RELEASE HIS HAND IN TIME AND PINCHED HIS HAND BETWEEN ROOF AND ROOF PLATE.
