He was bolting 7-8 crosscut. He let the head down and said the joy stick stuck causing the head to come down on his foot and ankle. It caused a sprained ankle.

Employee was installing a 6' tension bolt, when he was pushing it up to roof it started to bend and he spun rotation, causing bolt to spring around and hit his arm.

Employee was hit in face with bolter wrench. Employee had just finished put in roof bolt. Had wrench in hand left foot jack down. Canopy hit wrench, struck employee in face.

Employee was roof bolting he bent a bolt and it stuck in chuck when he dropped head bolt sprung out of chuck and hit his forearm.

Spun bent roof bolt hit right wrist.

Drilling anchor holes for tail piece, drill bit caught in creek.

Bolter operator was trying to align 2nd drill steel up with 1st drill steel and pinched his finger between the 2 steels at the coupler.

Miner operator got between the right rear corner of the machine and the rib while tramming the machine. He said the bits caught the rib, kicking the machine towards himself, smashing his right leg.

Employee was straightening roof bolt to put in drill, pushed feed lever up & mashed 4th finger on rt. hand between drill head & plate.

Employee operating dual boom bolter. While running roof bolter, dropped head on left foot. Smashed on top of metatarsal. Broke bone in top of left foot.

Employee was resting left arm on top of bolter ATRS, started bolter and ATRS raised to the roof. Left arm was crushed between roof & top of ATRS. Broken left arm above elbow.

Was straighting out roof bolt to install, pinched end of little finger between bolter and bolter head.

TRAMMING BOLTER FROM IN DECK AND MACHINE SLID AGAINST RIB AND SMASHED FINGER BETWEEN DECK AND RIB.

BOLTER OPERATORS STARTER STEEL WAS STUCK IN TOP. HE ATTEMPTED TO DROP DRILL HEAD DOWN TO PUT HIS DRIVER STEEL BACK ON. WHEN STARTED STEEL FELL OUT OF TOP, STRIKING HIM ON HIS LEFT HAND

Bolter operator was on his knees operating roof bolter when a rock fell from the top & hit his back & then landed on his leg.

Track bolter helper was helping bolter operator pull a stuck drill steel out of a drilled hole. A piece of bad top came down landing on helper's back forcing him down to the ground.

WHILE ROOF BOLTING, A PIECE OF ROCK FELL FROM A CLAY VEIN AND HIT OPERATOR ON HIS BACK.

While operating roof bolter the operator raised foot jack which allowed the front of the machine drop down. The operator had his foot under that part of the roof bolt machine.

WHILE ROOF BOLTING A PIECE OF HEAD COAL FELL FROM ROOF AND LANDED ON OPERATOR ANKLE.

SMALL ROCK FEL OUT OF TOP AND STRUCK OPERATOR ON TOP OF RIGHT FOREARM. THE ROCK HAD SHARP EDGES ON IT AND IT CUT HIS ARM.

ROOF BOLTING, DRILL STEEL FELL OUT OF TOP, SMASHING FINGER.

THE EE WAS ROOF BOLTING WITH HIS HAND ON THE DRILL HEAD AND CAUGHT HIS HAND BETWEEN THE DRILL HEAD & CANOPY.

EE WAS ROOF BOLTING ON THE TRACK & HAD TROUBLE WITH THE SECTION AND ONE OF THE MEN STARTED TO RAISE THE DRILL BOOM TO TAKE THE SECTION HOSE OFF & CAUGHT THE EE'S HAND IN BETWEEN THE BOOM & THE FEED JACK FRAME

BOLTER OPERATOR WAS ROOF BOLTING IN THE 6 RIGHT X-CUT ON 1 EAST SECTION. HE POSITIONED LEFT HANDON TOP OF ROOF PLATE TO STEADY IT AND CATCH OFF-SET IN TOP. OPERATOR DID NOT RELEASE HIS HAND IN TIME AND PINCHED HIS HAND BETWEEN ROOF AND ROOF PLATE.
