Two EEs lowered a hydraulically driven pump into a tank. The pump was suspended from a 2 ton crane mounted on the back of a welding truck & the pump's discharge hose was tethered to a fork truck. EE was on top of the tank to manage the position of the pump. The static thickener rake pulled the pump discharge hose which caused the crane boom to spring & hit EE's leg.

Showing all 5Employee was using auger on #1018 Mack Powder truck to load boreholes on the drill bench. After loading a hole he attempted to swing the auger toward the front of truck. He pulled the lever in the wrong direction. This caused auger to travel in wrong direction, toward him. Auger brushed against his arm, he jerked away, scraped across valve housing, caused laceration, 10 sutures.
Employee was using a torch to cut bolts from #6014 dozer track pads when a particle of dirt got under his eye protection and entered his right eye. The employee was taken to Epworth Conenient Care where he was given a prescription eye wash and released to full duty to return to work.
MECHANIC WAS CUTTING BOGIE PINS WITH AN ACETYLENE TORCH AND LEFT HAND WAS BY HANDLE OF TORCH. THE HANDLE OF THE TORCH WAS LOOSE ALLOWING OXYGEN TO BUILD UP IN THE GLOVE AND IGNITE.
EE WAS IN THE PROCESS OF INSTALLING THE FRONT DECK ON THE EUCLID R-190 TRUCK. A CRANE WAS BEING USED TO PERFORM THIS TASK. WHEN THE DECK FREED, IT SWUNG SLIGHTLY TO THE LEFT PINCHING EE'S LEFT HAND AGAINST THE HANDRAIL. (NO FRACTURE, CONTUSION ONLY).
