Keeper bolt on the cylinder pin at the rear of the moldboard broke allowing the pin to work its way out. When the pin fell out, it allowed the moldboard to drop, stopping the motor grader.

CREW WAS REMOVING TIRE CHAINS FROM DRILLING RIG. EE WAS CROUCHED DOWN BESIDE TIRE UNHOOKING CHAINS. THE VEHICLE WAS DRIVEN FORWARD ROLLING OVER THE TOP OF EMPLOYEE'S RIGHT LEG, CRUSHING LOWER LEG UNDERNEATH. THE LOWER RIGHT FIBULA IS FRACTURED.
