THE OPERATOR OF THE ROOF BOLTER SWUNG HIS DRILL BOOM TOWARDS THE RIB. HIS FOOT BECAME WEDGED BETWEEN THE BOOM AND SHALE ROCK. HIS FOOT WAS BROKEN.
