On April 13, 2014, employee reported lower back strain after jarring back while operating bulldozer. Physician issued lifting restriction on April 21 that prevents employee from fulfilling normal duties.

A bulldozer operator on a D10R Caterpillar dozer went over an approximately four foot tall steeply sloped bank. As the machine proceeded over this bank, the front end cantilevered and dropped suddenly and struck the ground, jarring the operator.

He was referred to physician who prescribed physical therapy for treatment (3/29/05). This employee has not missed any work time or been restricted as a result of this injury.
Employee was operating a dozer when a rock came off the highwall (right side). The rock came through the door glass over right leg striking the employee in the left ankle. This employee notified employer on 11/18/04 that he was pursuing physical therapy (treatment option).
