Employee was tramming the dual boom bolter. He reached out to grab his cable and caught his hand between a pallet of block and the roof bolter.
Employee was spotting bolts in a loose roof area, along the rib. The drill steel helped dislodge a piece of rock that slid onto the driller's canopy and then off it and onto his left ankle. Rock was 27" long x 14" wide and 0-2" thick (employee worked restricted duty. Dr visit 1/4/08 another MRI showed fracture. Employee off).
While installing a roof bolt, left index finger got caught between roof bolt plate and drill head of roof bolter.
Employee was installing a roof bolt. While straightening the bolt the bolt slipped and the employee fell forward hitting the bolt with his mouth. He dislodged 2 top teeth and 2 bottom teeth.
While pushing up roof bolt employee placed hand on top of coupler and pushed wrong leaver causing bolt to cut left hand index finger.
WHILE PUSHING BOLT & PLATE UP TO THE ROOF BOLT, IT KICKED OUT & STRUCK BACK OF LT. HAND CUTTING RING FINGER & MIDDLE FINGER, REQUIRING STITCHES.
Ee was bolting when a piece of rock dislodged along the rib striking him on the hard hat and left shoulder. The rock measured 2" thick, 1" wide and 3' long.
EMPLOYEE WAS INSTALLING CABLE BOLTS WITH A DUAL BOOM BOLTER. WHILE PUSHING CABLE BOLT INTO THE ROOF THE EMPLOYEE EXPERIENCED PAIN IN HIS LEFT SHOULDER. EMPLOYEE DID NOT MISS ANY WORK UNTIL 8/24/04 ON WHICH DATE HE RECEIVED SURGERY ON HIS SHOULDER ROTATOR CUFF.
PRIOR TO MOVING TO THE NEXT PLACE TO BOLT THE OPERATOR & HELPER SHUT DOWN AND TALKED. THE OPERATOR STARTED THE MACHINE UP & DROPPED THE ATRS NOT KNOWING THE HELPER SAW HIS BOLT MEASURING ROD A GAINST THE FACE. WHEN THE T-BAR WAS RELEASED IT HIT THE HELPER IN THE LEFT BACK & SHOULDER AREA KNOCKING HIM FROM A CRAWLING TO A PRONE POSITION.
WHILE ATTEMPTING TO INSTALL A ROOF BOLT THE EMPLOYEE HAD A HOLD OF THE BOLT AND PUSHED THE WRONG LEVER SPINNING THE BOLT AND THE PLATE STRUCK THE EMPLOYEE ON THE RIGHT WRIST.
WHILE PUSHING A ROOF BOLT INTO THE ROOF IN A FALL AREA, USING A LONG BOLT TIGHTENING WRENCH THE OPERATOR WAS SPINNING THE BOLT TO MIX THE RESIN WHEN THE WRENCH CAME OFF THE HEAD OF THE BOLT AN D STARTED TO FALL WITH A CIRCULAR MOTION. THE OPERATOR STUCK HIS LEFT HAND OUT FOR PROTECTION AND THE STEEL HIT HIS HAND
DURING BOLT INSTALLATION THE MAN STRAIGHTENED THE BOLT INSERTING THE HEAD OF THE BOLT IN THE CHUCK OF THE BOLTER. HE STARTED SPINNING THE BOLT WHICH EVALENTLY HAD A BURR, WHICH CAUGHT HIS GLOV E AND WRAPPED HIS LEFT HAND GROUND.
WHILE BACKING MINOR OUT OF X CUT PINCHED FOOT BETWEEN MINER SEAT AND RIB.
THE EMPLOYEE WAS INSTALLING A 42" RESIN BOLT AND PLATE IN THE DRILLED HOLE. HE PUT 1/2 OF THE BOLT IN THE ROOF THEN STRAIGHTENED IT BY HAND, PUT THE HEAD OF THE BOLT IN THE CHUCK, STARTED TO P USH IT IN THE ROOF AND SPIN IT TO MIX THE RESIN.HIS CUFF ON HIS GLOVE CAUGHT THE PLATE AND TWISTED HIS HAND AROUND THE BOLT.
EE WAS PUSHING A BOLT UP WITH A BOLTER HEAD AND GOT FINGER BETWEEN HEAD OF BOLTER AND BOLT. CUT FINGER.
WHILE ROOF BOLTING KNEE SWELLED UP AND GOT SORE.
VICTIM WAS INSTALLING ROOF BOLTS. PINCHED FINGER BETWEEN BOLT PLATE & BOOM OF ROOF BOLTER.
EMPLOYEE WAS ROOF BOLTING AND GOT FINGER BETWEEN BOLT PLATE AND DRILL HEAD OF MACHINE.
