While grinding a wheel motor commutator, the grinding stone wedged in the commutator causing the grinding stone to spin around pinching the left index finger between the grinding stone and the commutator. The pinching caused a laceration requiring 5 sutures.

Employee was air arcing a heel band off of a 4100 shovel bucket. He was a making a cut with the air arc when the band fell from the bucket landing on his right wrist and hand.

A shop welder was needle scaling a welding bead on a haul truck. He felt something in his eye and rubbed it. He contacted mine rescue personnel. After attempts were made to flush the eye, the employee was taken to an optometrist. Extensive scratching on the eye surface was found. The employee was given a prescription antibiotic in the eye and a full duty release to return to work.

While using a hydraulic cylinder to straighten bent flanges on a dragline mast, the cylinder dislocated, sprung from position and struck 2 employees approx. 8 feet away from the cylinder in their protective safety headgear. Employee 1 recieved a minor contusion to the forehead and Employee 2 recieved contusions, cuts and fracture from impact of the cylinder to his safety glasses

EE was cleaning a weld on a table leg when the grinder kicked back and lacerated left fore arm. Employee was taken to hospital and recieved 8 stitches. Returned to work on next scheduled shift.

Task to remove cable guide of "A" frame, stagged in shop, when partially cut loose by air arcing, cable guide broke loose and dropped about 16 inches to floor, striking welder on top of right foot as he stepped back and rested on steel toe of left boot.

EE was using the hydraulic press to flatten a push plate. He had set the plate and was pressing when spacer dislodged and hit him in the face. He has large cut across right cheek bone and upper jaw is dislodged from skull.

While a mechanic was performing the task of cutting safety glass, methanol vapors ignited burning the mechanic's left arm and hand. The mechanic incurred 2nd degree burns to his left hand and forearm and 3rd degree burns to the finger tips of his left hand. The mechanic was transported to a medical facility and did not return to his next scheduled shift.

EMPLOYEE WAS GRINDING OVERHEAD INSIDE THE DRAGLINE TUB EVEN THOUGH HE WAS WEARING A FACE SHIELD HE THOUGHT THAT HE HAD GOTTEN SOMETHING IN HIS EYE. HE USED FIRST AID PROCEDURE TO REMOVE IT. LATER THAT DAY HIS EYE STARTED TO BOTHER HIM & HE WAS TAKEN TO AN OPTOMETRIST WHERE A RUST RING WAS REMOVED.

A P&H shovel bucket jarred haul truck #2037. The employee was seen at the hospital and given a work release. One week later, returned to the doctor and received another work release. On 3/26/06 he again went to the doctor and received 3 weeks of physical therapy. This employee is presently under JRM investigation for falsifying pre-employment medical history.

EE BACKED HAUL TRUCK #2082 UNDER THE 1214 XPB SHOVEL & RECEIVED THE 1ST BUCKET LOAD OF MATERIAL (120 TONS). ON THE 2ND PASS, THE OPERATOR WAS JOLTED BY THE MATERIAL BEING LOADED INTO THE HAUL TRUCK AND SUSTAINED LOWER BACK INJURY. SECOND BUCKET OF MATERIAL WAS ALSO 120 TONS OF OVERBURDEN. OPERATOR WAS WERAING HER SEATBELT.

Employee was working on heater system on a Rubber Tire Dozer. His left middle finger came into contact with the fan blade of this system while the fan was running. He received 14 sutures to repair the laceration.

While tighting lug nuts on a piece of earth mover equipment the impact wrench slipped and caught his finger between the impact wrench and wheel causing an open tuft fracture of the right ring finger.

Drill crew was air developing a previously drilled and cased 5" hole. Approx 160' of 1" t & c steel line was suspended in the slips on the rig table. The slotted wrench tipped allowing 1" line to fall about 1 1/2 ft to the table. The 2" elbow adaptor caught ee's finger between table and adaptor requiring stitches.

A #2067 haul truck was backing under the 1214 shovel when the door to the bucket opened accidentally. The operator was jostled in the cab of the haul truck causing soft tissue injuries to his lower back. The physician prescribed 3 days of restricted duty activity. He returned to work with a full duty release on his next scheduled shift.

EE STATES THAT WHILE MOVING A JACK HAMMER HIS RIGHT INDEX FINGER BECAME PINCHED BETWEEN THE JACK HAMMER AND SECTION OF HANDRAIL. EE SUSTAINED A LACERATION TO THE TOP OF HIS FINGER AND A SEVERE D TENDON THAT REQUIRED SURGICAL REPAIR AND STITCHES.

THE IMPACT WRENCH ENGAGED DUE TO A LOOSE PLANETARY GEARING HOUSING AND PIN CHED THE EMPLOYEES LEFT THUMB BETWEEN THE BRACKET AND THE WRENCH SOCKET. THE EMPLOYEE RECEIVED 6 SUTURES AT THE E.R. ROOM & CAME BACK TO WORK WITHOUT ANY RESTRICTIONS.

HE WASHED A MET AL SPECK OUT AND THEN WENT TO THE HOSPITAL AS ITWAS STILL BOTHERING HIM. NOTHING ELSE WAS FOUND IN HIS EYE BUT THEY ADMINISTERED PRESCRIPTION EYE DROPS.

EMPLOYEE WAS DOZING ALONG THE HIGHWALL AND DROPPED INTO A HOLE THAT WAS 2-3 FEET DEEP. THE INJURY RESULTED FROM HIS HOSE PUSHING AGAINST THE DOZER CONTROL HANDLE WHICH PINNED HIS FINGER AGAINS T THE TOGGLE SWITCH FROM THE IMPACT OF THE DOZERSETTING IN THE HOLE. HE RECEIVED A SMALL PUNCTURE LACERATION WHICH REQUIRED 5 SUTURES TO CLOSE THE WOUND.

EN EE WAS REPAIRING A CRACK ON THE FRAME OF A 190-TON HAUL TRUCK. HE HAD POSITIONED HIMSELF BELOW THE CRACK WELDING OVERHEAD. HE HAD COMPLETED A PASS AND USED A METAL CHIPPED TO REMOVE SLAG. H IS WELDING HOOD WAS DOWN. HE FLIPPED HIS HOOD UP TO INSPECT THE WELD. HE BEGAN CHIPPING SLAG WITH HIS HOOD UP AND HIS SAFETY GLASSES ON WHILE CHIPPING. A PIECE OF SLAG IMPACTED HIS RIGHT EYE.

EMPLOYEE WAS PUTTING SCORIA DOWN ON THE ROADS WITH A SCRAPER AND HIT A ROUGH SPOT IN THE ROAD. SHE WAS SEEN AT THE HOSPITAL AND DIAGNOSED WITH A BACK STRAIN. SHE CONTINUED TO WORK HER FULL DUT IES, BUT HER BACK CONTINUED TO CAUSE HER PAIN. ON 8-1-01 SHE WENT BACK TO SPECIALIST AND WAS TOLD TO STAY OFF EQUIPMENT IF HER BACK WAS TO HEAL. SHE BECAME A LOST-TIME AS OF 8-02-01.

THE JUG TO THE # SWINGER HAD TO BE RE-ALIGNED AND REPAIRED. THE JACK WAS PLACED BETWEEN THE WALL AND THE SWINGER. WHILE EE WAS NEAR, THE JACK SLIPPED OFF THE WALK, STRIKING HIM IN THE LEFT LEG AND LEFT HAND.

TWELVE FEET T O THE GROUND. THE INCIDENT BECAME REPORTABLE ON ON JAN 11,2001, WHEN THE EE SOUGHT MEDICAL ATTENTION FOR A CHIPPED TOOTH.

THE INCIDENT OCCURRED AS EE WAS USING AN IMPACT WRENCH TO TIGHTEN THE BOTTOM ROLLER CAP BOLTS ON A CAT D 11 ROLLER FRAME. TO ACCOMPLISH THIS TASK, EE WAS LYING ON THE GROUND ALONG SIDE THE ROL LER FRAME, WITH HIS HEAD DIRECTLY BELOW THE OPENING TO THE GUIDE ASSEMBLY HOUSING. THE VIBRATION FROM THE IMPACT WRENCH CAUSED THE GUIDE ASSEMBLY TO SLIDE OUT OF ITS HOUSING STRIKING IN FACE.
