Victim was taking "Dog Bone" out of head drive. Head drive shifted and caught him between motor and pole.
EE WAS INSTALLING A ROOF STRAP ACROSS THE MINE ROOF. WHILE ADJUSTING THE STRAP, A PIECE OF DRAW ROCK FELL FROM MINE ROOF AND CAUGHT EE'S HAND ON TOP OF CANOPY, SMASHING HIS MIDDLE FINGER (L).
SHEARER ENCOUNTERED A CORE HOLE FULL OF WATER. WHEN CORE HOLE WAS HIT, WATER CAME OUT & HE TRIED TO DODGE THE WATER AND FELL RESULTING IN INJURIES.
INJURED WAS ASSISTING MINER OPERATOR, WHEN A PIECE OF DRAW ROCK FELL FROMT HE ROOF STRIKING HIM ON HIS RIGHT LEG. THIS RESULTED IN A FRACTURE TO HIS RIGHT ANKLE.
EE WAS OPERATING THE ROOF BOLTER WHEN A ROCK FELL FROM BETWEEN BOLTS, STRIKING EE ON HEAD & LEFT THIGH.
